Hi.

My problems started over 10 years ago when I was only 19 years old. I'm not sure what (or how it) happened, but it started after a night of rough sex. Already on the next morning I noticed I had a "hard flaccid": my previously completely soft and elastic penile tissue was now constantly in a state akin to what you'd normally get after being in cold water. Retracted towards the pubis with diminished blood flow, causing the hardness while flaccid.

After this my morning erections were gone and have been to this day; although it clearly isn't hormonal since many a morning I can feel how my organ "would like to" be erected, but it's filled with blood just slightly, not qualifying as an erection. Arousal-induced erections became more difficult to achieve, but it was possible at first, although compromisedly. I noticed my penis had shortened a little, and there was this feeling of pressure. It hasn't gotten better throughout the years but worse. Now I just don't seem to be able to get an erection anymore. What I get is like 30% of an erection, and most of the times only if I stimulate it manually. Some blood manages to get there, but it stays too soft, and the erection goes away quickly.

Okay, so what makes me resonate with Peyronie's especially, is because there has been this lumpy hardness running through my penis since the beginning of all this. The "hard flaccid" symptom of corpora cavernosum has subsided over the years, but the "fibrotic"-like inelasticity along the spongiosal, and/or septal tunical part throughout the "middle" of the penis has never went away.

I've been to 3 urologists over the years. Pretty much the psychosomatic suspicions-type of a deal. They don't seem to understand what I'm trying to describe to them. But if this wasn't a real physical problem, I wouldn't have hung on to this sort of an obsession for 10 years, especially considering that I was completely content with my penis before this happened.

One of the urologists (presumably thinking Peyronie's) ordered an ultrasound (I think it was a Doppler, but I'm not sure) and the conclusion was: no plaques detected, tissue is smooth and regular. And I don't think there should have been plaques in the outer organ itself, because it feels like "the root of the problem" (no pun intended) is that there's some fibrotic scarring somewhere at the root of the penis, making it impossible for the erectile tissues to relax adequately for an adequate erection (I can literally feel the hardness there, although it may not be noticeable to a doctor palpating it, because he doesn't have my "previous" dick to compare it to, to deem it changed) --- (although, now, after all these years my penis has become more "lumpy" overall and there are more of these hard "rings" around the soft tissue. This I believe to be secondary to the inadequate blood flow and lack of constant erections that has been going on for too long, so now it has begun to affect the penis overall; sort of like premature aging of penis, if you will)

I've read discussions and articles about "hard flaccid syndrome" and "chronic pelvic muscular tension", but it doesn't seem to be at the root of my problem, because many of those supposed "hard flaccid"-sufferers report how the hardness comes and goes depending on the posture etc., making it clearly more of a muscular problem. But the hardened cord running through the middle (septal?) part of my penis has never went away.

I don't have a noticeable curve to the penis. But if Peyronie's is defined as any fibrotic change to the penis, then it is the only condition that has any resemblance to my problem, even though my fibrosis hasn't been objectively detected (yet?)

So this was my introduction, and I'm not sure how coherent it was. My reason for finally signing on this board was to ask this:

After 10 years have gone by, is there anything that could still be able to alleviate the fibrosis? (at least theoretically) I've dabbled with Pentox (it's prescription-free here) but not for a long enough time to judge. The urologist who gave me the Ultrasound almost 10 yers ago prescribed me Cialis, but I didn't understand these things then: I thought it would be helpful only "functionally" to be able to have an erection for sex (which I still at that point managed, even though it was more difficult already) and the medicine cost too much for my budget for that purpose only. Only now, years later, have I found out that PDE5:s can also be used therapeutically...

What changes have you noticed?  Curves, shortening, hourglassing, ED, etc?
PxD 2 yrs 9/16.  Failed all treatment. 9/11/18: excision, grafting & implant Dr Karpman MtnView Ca, AMS CX 18cm + 3-1cm RTEs.
Pump failed.  2/11/20 Dr Karpman installed Titan 22cm +1cm RTE.

So, to sum it up:

-Palpably noticeable induration running through the penis centrally, which I can only surmise has something to do with septal tunical hardening (?)
-Shortening of the penis due to this inelasticity preventing adequate relaxation of the erectile tissues
-Erectile dysfunction, no morning erections and arousal-induced erections are hard to achieve; when they occur, they are of weaker quality and go away pretty much immediately without stimulation -- this also seems to have something to do with the compromised function of muscles and nerves due to the tissue damage, causing the blood to leak out
-All this comes with increased sensation of pressure in the penis, as if it was "pulled" too tightly back towards the pubis
