Concerned about Implant Surgery with Acute Peyronies and Strictures

Developed peyronnies two years ago after radical prostatectomy. Have been on pentox and ciallis/ viagra. Urologist is recommending grafting of hourglass near base of penis and to correct 50degree bend to the left. He is also recommending penile implant at same time.I am still experiencing burning  and pain in penis and so am wondering wether I should be going ahead with surgery if  I am still in acute stage. Urologist says there is differing opinion when surgery should be performed but after two years i should be reaching chronic stage. It doesnt feel like it.
I am also concerned due to some strictures  that had to stretched in my urethra ( a result of many years of numerous cystoscopies having had chronic prostatitis and benign prostate enlargement) that i will further damage my urethra with further surgery. Urologist says there is always a risk.
Have any of you had this situation and still went ahead with surgery?

Hey Rob. Couple of questions First:

-where are you located?
-how interested are you in having an implant? Just following surgeons advice? Or thinking it would be a feasible treatment for you?
-have you read the erectile dysfunction and peyronies guidelines, from the American urological association? And your countrie's equivalent, if you are not in the US?

If things are functional, you can get erections and have intercourse, I would think long and hard before considering any surgery.

I have not had an implant, although might be my next step, but I have had 2 operations with very disappointing results, to the point I am in a worse situation than I was before surgery

Don't rush.  It is important to know that if you have ED and deformity that a very good surgeon can correct your deformity with just the implant and no additional surgery.  That is just a fact.  Your first step should be to find a surgeon who is accessible to you that has good results with that.  The healing is much faster, cheaper, and less chance of an infection which is the biggest concern.

You might want to check around. My doc never asked me what stage was in or how long. I had Peyronies Disease, hourglassing, plaque, etc. About 35 degree at surgery. In fact not one surgeon I saw asked me about stages. All 4 were you got peyronies, you need a titan, when do you want to do it. Thst was it.

They said implant would do it by itself. For me it did. Right after surgery, straight and hard. It just got better as each month went by. He did no scraping or anything except implant.

I am having an issue now with the reservoir but not sure why. Does not really matter, getting this fixed or another put in asap.

I got used to over the past 10 months having a good erection for anything I wanted it for and am getting it back.

You just need to decide what you can live or not live with. If I never got hard again, so be it, but i am damn sure going to try my best options so I can say I did all I could.

Thanks for replies . I needed to check out Merrix pdf about implants and other info before replying. I am in Australia and there arn't many urologists who specialise in grafting and implants over here. Maybe about 3-4 some of whom dont have many recommendations. Intercourse is becoming increasingly difficult due to the side bend. If the implant could be done without grafting i would proceed, but the grafting sounds plagued with risks such as infection, shortening and further pain. I think i will seek another opinion which will require interstate travel as to wether additional grafting is really necessary. The travel doesnt bother me except i would have to find some accommodation without assistance for a few weeks. Has anyone had grafting and the outcome ? Specific results would be appreciated if you dont mind sharing.

I had a 90 degree bend and hinge. I had grafting with a modified Nesbit. I lost almost 1.5 inches. 3 years later, I had an implant. I interviewed 3 urologists before having the surgery and none mentioned an implant. I would consider an implant over grafting. Don't stop searching until you speak to someone that does lots of implants. Although I am still shorter than my pre Nesbit surgery, I am thicker which is great.

If you are fortunate enough to be among those that can afford surgery out of the country, Dr. Eid in Manhattan NY is a doctor who will definitely make you straight with an implant with no loss of length.

Thanks guys, I feel a lot more informed. There is one doc in another State who has being performing implants since the 1980's so I 'll check him out more and get a second opinion. If that doesn't work out I'll have to decide wether to go to America.

I feel for you. I went to several urologists before I had one to even discuss Peyronies Disease. I also interviewed several surgeons that do implants. The key is to find the best urologist that does a lot of implants. I was fortunate to find mine at Duke Hospital in a neighboring state. Don't give up your search. I love my new penis and so does my spouse. I am so glad I didn't give up. It's a difficult subject, but eventually you will find a surgeon that will say, "oh yeah I can fix that, and it will correct the bend, the hourglass and even improve your girth.

I had both the excision, grafting and implant.  It went well, but if a highly respected, high volume doc could do it with just the Titan implant and hand modeling I would go that route.
PxD 2 yrs 9/16.  Failed all treatment. 9/11/18: excision, grafting & implant Dr Karpman MtnView Ca, AMS CX 18cm + 3-1cm RTEs.
Pump failed.  2/11/20 Dr Karpman installed Titan 22cm +1cm RTE.

Tony, never asked the reason and this might be material for a different thread, but just curious why you needed excision and grafting with the implant? Is it because it was AMS and not as rigid? What was surgeon's reasons?

Hey Rob,
I to have peyronies with a 55 deg upward bent with hourglass. I have been talking to a urologist in brisbane by the name Eric Chung who wants to do a graft, excision  with a implant due as he puts it a better result. He was recommended  by an other urologist and appears quite skilled.  He uses both brands but doesn't recommend one over the other. Im now at the stage of maybe just installing the Titon due to its  better hydraulic strength, with far less risk of completion.
52 years old, Peyronies since 2015, 3 rounds of Xiaflex. Currently doing traction and VED, looking to get a Implant.

Get the titan. It will take care of it. Took care of my bend and a real bad hourglass and plaque the size of my thumbnail. And I got big hands.

Dealing with a revision now due to a hernia surgery affecting my titan. Loved my titan, fixed all my issues. Right up till hernia surgery fd it all up.

Titan took care of it all period. Just get it and cycle it daily as long as you can stand it.  And if you ever get a lower groin hernia make damn fin sure your surgeon knows where your tubes and reservoir is.
