Peyronie's, Prostatitis, or both? Please help.

Hey All,

I've been lurking on here for a couple months after the development of my symptoms but finally decided to make a post. I'm hoping to get some advice and/or insight from the community here as to what is going on with my situation and what my next step should be. I saw a urologist yesterday and was diagnosed with a form of Prostatitis as well as "possible Peyronie's." I asked for a Pentox prescription but the doctor refused. She said that "we can talk about Pentox" in a month, once I finish the Bactrim prescription.

Here is my story, which I will try to make as brief as possible. 27 years old currently. In 2011, I twisted my penis downward and to the right while masturbating with my left hand and trying to ejaculate into the toilet while seated on it. There was no pain. Immediately after ejaculation, I noticed my penis hanging at an angle I had never seen before. I also began to notice I had slight curve while erect (maybe 10 degrees) and slightly weaker erections following this incident. The curvature seemed to come from the physical force of my twisting my penis, rather than from rapid or spontaneous development of plaque. I then abstained from masturbation for a long time in the hope that the issue would fix itself. It did not.

Two years later in 2013, I began having steady sex. Erections were not great, maybe 5/10. I would sometimes feel a dull, aching pain in my bottom right part of my shaft after sex (the area where my largest plaque is now located). This would subside after a few days of rest/abstinence. Since 2015, I also have experienced intermittent stinging pain (1-5/10 on the pain scale) in the upper left part of my shaft, which is worse when wearing tight pants or jeans, as this area rubs against the pants. This would often subside for months at a time only to resurface for a week or two.

I have been varying levels of increased pain in my penis since October (1-10 on the pain scale). I was masturbating (relatively gently but against the curve with the right hand) after smoking a spliff and sharp, stinging pain (10 on pain scale) began immediately upon ejaculation, it seemed to be located primarily in the areas on my penis where I have identified plaques and/or suspect that new plaque is forming. Pain was very bad in October and early November. I remember feeling like my entire penis was on fire and/or cracking at times. I have three palpable plaques, two on the right side (one near the base and another slightly below the glans) and one on the left side near the base. I also have circular denting/hourglassing beneath the glans, which is most apparent when I am flaccid or when I get out of cold water and the penis is trying to refill with blood. Basically, the section of my penis just below the glans can at times appear much less full than the glans and the rest of the shaft. Other times it looks normal. I also believe other, small plaques are forming near the base and upper left of the shaft, as the skin in these areas is slightly raised, they frequently have stinging pain, and are sensitive to touch when flaccid or partially erect. Erectile function is moderate. I feel as though my penis often is not as hard as it should be when having sex and I sometimes lose erection strength during sex, especially when trying to pump quickly.

As I said before, I have a slight curvature (maybe 10 degrees) but I do not believe it is plaque-related. It seems to be a result of the 2012 twisting, as the curve has not changed much, if at all, since then. Plaques seem pretty soft, although one of the ones at the base is tender to the touch and sometimes hurts after sex. I've also experienced temporary numbness on a portion of the right side of my shaft twice during the past four months. I've been taking L-Arginine daily, Al-Car on and off, and around 2.5 mg Cialis before bed. I tried traction, and it seemed to greatly improve by erection quality, although the glans did not seem as full, I was rock hard from the base up. Traction also seemed to increase the pain/inflammation, so I decided to wait until getting Pentox before continuing it. I've been cutting sugar and carbs, and am pretty sure I've been in ketosis for the past couple weeks. I have not seen much change in pain or symptoms.

I still have penile pain/stinging (usually 4-5 on the pain scale). The penile pain/stinging gets worse when I am sitting down. It is especially bad while in class or on long drives. Walking and light exercise seem to relieve my symptoms temporarily. I have very slight difficulty with urination, but am not sure what to attribute this to. Last week, I began feeling a cold, almost tingly feeling in my feet and ankles. My feet are very cold to the touch when this is happening. I have never had this before, and thus don't believe it is neuropathic. I am attributing it to the introduction of Cialis or the removal of carbs from my diet (could also be symptom of Prostatitis?).

I would greatly appreciate any advice on what my next move should be. The choices are: 1) follow my uro's recommendation, take the Bactrim for prostatitis, continue the Cialis/L-Arginine regimen, and revisit Pentox with her in a month or 2) Pay out of pocket to see another urologist or a specialist who will hopefully prescribe me Pentox, and began the PAV cocktail along with traction and VED as soon as possible.

I don't think holding off on the pentox would do much harm especially if you keep up what you're doing and add traction or VED.  But if it's going to bother you go ahead and have someone else prescribe the pentox—my primary was willing to do so.
PxD 2 yrs 9/16.  Failed all treatment. 9/11/18: excision, grafting & implant Dr Karpman MtnView Ca, AMS CX 18cm + 3-1cm RTEs.
Pump failed.  2/11/20 Dr Karpman installed Titan 22cm +1cm RTE.

Thanks for the response Tony. I just want to start VED and resume traction as soon as possible. I read a study where it said VED without Pentox can cause further injury/inflammation, and traction seemed to cause more inflammation/pain when I tried it, which I'm hoping Penox will help with.

I've been experiencing pain since October, and It took me a month to see the uro. I guess I'm just worried that letting another month go by while I seem to be in an acute/active phase will cause further damage. Maybe I'm overthinking it.

phew that's complicated but you have a good observation and clear head which is good further down the road.
This will take some time..

I wonder why the prostatitis? Sounds to me like your penis and pelvic floor out of whack might contribute to that. It's all basically sitting at the same place and connected.
If that was the case the prostata might not calm down if your penis and PF isn't sorted..
It could be nerves or veins or muscles are strained or clamped off which then aggregates the prostata.
Why no pentox while Bactrim? No good in combo?

In any case. In that active situation you should refrain from anything that could make matters worse. Masturbation and intercourse you should be conscious of bending and overdoing it. Same goes for traction and VED.(if you do start with very low intensity and time)

The initial incident probably put your pelvic floor, the fascia and/or the muscles, out of whack. That could've led down the line to further situations that inflamed parts that resulted in plaques.
Btw. think about distributing your cialis and later the pentox throughout the day cause it wont be active all day due to it's half life.

Think about seeing a pelvic floor specialist and maybe check if your hips etc. are aligned right(chiro or physio).

The erection problems are most probably related to your plaques, it messes with the blood flow and expansion.
If you have established plaques and no active inflamation going on you can with smart and patient use of traction, VED, supplements(maybe transdermal like I do) get your situation better.

Hey OP. Your case sounds incredibly similar to mine, which started last spring as a nasty bout of prostatitis. Yours sounds like more of a pelvic floor dysfunction, which a good pelvic PT can help you with. In regards to the penile pain, you probably have some inflammation going on. I'd try and get back on Pentox/PAV and focus on overall health for a while. I was in your spot last summer and I went crazy trying to find an overnight fix.
