Implant surgery after failed incision and grafting

I'm still early days into my recovery from an incision and grafting surgery.  It's been 9 weeks, but I have gone from a 100 degree curve to a post op 40 degree curve.  I have also lost length due to the graft becoming totally hard and rigid.

I have a follow up with the surgeon in a few weeks, but just want to understand my options, before further discussion with him.

So, how can an implant help?  Can I have the previous graft removed?  Will the implant help to straighten my erection, and can it restore my ore op length?

I'm 46 and frightened of further surgery, as things just keep getting worse.

Look at the top of the implant board.  You will find an index of journals with brief descriptions.  You will see some that mention bends corrected by an implant.   Do yourself a favor and put the time in to read some of those journals.  It will be an education.

Few things are more important than getting a very good surgeon.

Hi MarkDS

Interested to know what way your curve is and is it from Peyronies or Congenital?

I had Congenital Curvature in a downward fashion. Then as I entered my mid 40s and my erection strength was not as good as before, I found I could have sex in more positions easier - but was a double edged sword as it turned out. I damaged my penis and a plaque formed (strangely enough on the underside which bent it down even more). Consequently, I had an excision of the plaque and grafting of the space left over. It was a disaster. I was left with a penis 1.5-2 inches shorter and the graft failed within weeks. The end result was even more horrific looking than pre-surgery. Not only did I have a 90 or more degree bend down the way again (like a banana) and 2 inches shorter, but also my penis now had a very obvious chunk out of it on the left side (indentation) just before it took a dive south. Even masturbating with this thing was a challenge.

Mr Mike Fraser, out of Glasgow Royal Infirmary, performed implant surgery on me on June 3rd this year and I'm now the proud owner of  a Coloplast Titan implant. Sure the 5 weeks have been a monumental struggle in the early 3 weeks, but like everyone else, I'm coming out the other side now and things are looking and feeling good. For the first time in my whole life I have a straight(ish) dick.

So I have been there with the whole excision and grafting and while it maybe great for some, it was a disaster for me and made my situation much worse. If I'd found Mr Fraser earlier, and jumped straight to implant as he has since said he would have done, I'd have been over a year in a better place.

.... and like Hawk has just said, the journals on here are an education and have helped me no end. The fact you are also from the UK, where access to a leading expert surgeon is more difficult, in addition to the fact that you had excision and grafting, makes my journal very relevant to yourself.

MarkDS, I'm where you are.  Failed incision and grafting, but I can no longer get an erection whatsoever.  Doc now says an implant is my only option.  I too lost significant length, based on stretch test, after having been told this procedure did not result in a loss of size.  One of the things you should consider is traction or vacuum as you weigh your options, as there's a lot of evidence doing so can restore you to pre-surgery length.  I've only been at it for a week, and have seen no improvement yet.  It takes a couple months as I understand it.  Go slow at first.
Peyronies for 2+ years.  Have ED for 8 months
Tried Xiaflex, 3 cycles, zero improvement
Had incision & grafting surgery, Feb 2019.  Fixed curve (I think) but resulted in substantial length & girth loss.
Facing implant as remaining option

Thanks for the replies.

I have been using VED, but this is really aggravating my circumcision scar, which is still quite painful, particularly the underside.

I also think I have a new lump, which I hope is not a new plaque.

I feel lost at the moment.

Just read your other 70ish posts on the forum, Mark. You too have been through the ringer with this bloody terrible disease. I also noticed you've had incision rather than excision. Sorry for not reading your first post properly.

Hey Mark, did you look into Dr Garaffa?

Yes, had a quick look.  I have an appointment with my GP next week.  I'm going to ask him what my options are, if anything is available on the NHS, and if so, can I choose who to be referred to

Hey MarkDS

Was wondering how you got on at the GP etc? Any progress?

Hi Roddy

My surgeon pretty much said it's unfortunate the results have not been as good as expected, but there is nothing further he can do.

I am seeing another surgeon in Harley Street on Friday for a second opinion.

I'm still using my VED, but no improvement yet.  I may try traction after seeing the surgeon.

I have also booked pelvic floor therapy in September.

So I've not given up all hope; yet.

I am able to get an erection, but difficult to maintain it.  Also lost sensitivity, I think due to the circumcision, so find it difficult to finish.

Worse issue I have is that penis retracts as I walk around etc, and then the tip rubs on my clothes and is really uncomfortable.

Still wish I had not opted for surgery and feel quite angry towards the surgeon for not adequately going through the risks.  I was led to believe that there would be little, if any, length loss, only risk of a residual curve of up to 30% , and circumcision would not affect the feeling during intercourse.  He was wrong on all three counts.

I'm very sorry to hear that, MarkDS. There must be other members on here that have gone through/are going through the same as you or at least some of the same symptoms. Would be good to hear their thoughts as you sound quite frustrated. I'd be surprised if the Harley St surgeon on Friday cannot help you. Let us know how it goes.

Hey Mark, so sad to hear what you have been going through!
Don't give up, I had several urologists the JHH at said they were sorry, but there was nothing they could do for me. Even though each situation is different, I believe your quest for help will succeed.
I know it's frustrating, but keep going until you find a Excellent doctor that can help you.
Prayers for you.

Thanks for the kind words.

It's been 5 months since surgery.  Still have s significant residual curve, but sensation has returned a little.

Still uncomfortable when walking around, but think it has improved a little.

I've been using the vacuum pump religiously, but not sure it's helping.

I've recently bought a Phallosan forte, but so far finding it difficult to use as it's really painful when the sleeve rolls up.  I will keep persisting with it, but not sure I could wear it all day at work.

Hi MarkDS

When you say it hurts when the sleeve is rolled up .... what/where exactly do you mean? Where abouts does it hurt to be exact? I've worn this device for 5 months and can maybe help you if possible?

Mark,

THe more I read about your "adventure", the more concerned I become.
We cant change the past, so the move must be forward.
I know it's frustrating, but I guarantee if the doctors that you have seen had the same issues as you, they would be more empathetic and try to find someone who can help you.

I know you have different healthcare and I don't pretend to understand it, but sometimes it's what you say and to the right person that gets the attention. As I had said I had to visit 3 different urologists before I started getting the down and dirty facts about Peyronie's Disease......And even at that point, I still didn't get the right treatment.

I guess the important questions to ask
Are you able to maintain an erection to climax?
Are you able to have sexual intercourse to climax?

I could get an erection with medications and injections, but they became less and less effective, until sex was becoming impossible. And that's what I said to the newest surgeon that gave me an implant.
This all plays into our feelings of manhood, our virility, etc. It is depressing and you must convey what's happening to the surgeons. Maybe you are, maybe I am just rambling on, thinking you are there, but are not.

I can't tell, did you ever go the other doctor that Roddy talked about? Roddy is a success story, I am a success story, others have been fortunate too, but we have had to fight and fight hard to be heard. I know it can be tough to fight when you are down, but don't give up, keep fighting and seeing as many GOOD urologists as you can.

Please hang in there, be blunt when you talk to urologists, and don't give up. If I am missing something I apologize, but I've been through this and I am so sorry you are going through this too.l

Thanks for the reply.

I can get an erection and I can climax, although I have been using viagra.

My main issue is that the graft is so rigid that my flaccid penis protrudes outwards Esther than hanging down.  It then rounds on my clothes and becomes sore, and then it retracts and is really uncomfortable.

If I could resolve this issue I would be happy(ish)
