Just started pelvic floor therapy last Friday to help heal post surgery.  Not everyone's cup of tea as a lot of it is internal, but has anyone else had this type of therapy?  My therapist says she sees a lot of men with peyronie's and ED

TDix my pelvic floor has been damaged from a young age and involuntarily spasms, causing Peyronie's problems (or used to). I also tightened the area on purpose thinking it was somehow good back then, since it makes your penis harder for a moment. I just had a long talk with Diehardpatriot about this.

Reverse kegels and biofeedback have helped me. For biofeedback, I touch the sensitive part of my penis while relaxing my pelvic floor, since I usually involuntarily kegel. Since a lot of guys have a problem requiring kegels to orgasm, they should train themselves off that and can use masturbation as a kind of chance for biofeedback and to relax the area.

How is it working for you?

I've only had one session, but she found that my right side internally was extremely tight.  She pinpointed certain areas to relax the muscles, and said after some more sessions I should notice less pain and numbness in my penis.  I always thought as you that we need to strengthen our Kegels, but when you have trauma like Peyronie's or ED in the pelvic area, you actually need to relax the muscles, which in turn will strengthen them.  During the therapy, I was involuntarily tightening my whole core and even my legs.  She showed me that those need to be relaxed to help heal.

Had my second session today.  She focused on any tender spots, especially on the transverse perineal muscles.  Still too early to tell if it is helping but I will say I have had less soreness in my penis, and my numbness from surgery seems to be improving

Hey Tdix,

Very interesting stuff. I myself tried pelvic floor therapy, but just 2 sessions and got discouraged. I may go back to it to cure my 2 years still acute phase. Please continue and keep us up to date!!
- 35 yo, Peyronies Disease with chronic pain, general hourglassing with girth and length loss since 09/01/17.
- Pain almost cured with Hirudoid cream and diet/lifestyle changes (see my topics on this); deformity still here, but partly reduced with traction + VED.

She also massaged the hip muscles that go into the pelvic floor, which she said were also really tight.  I think that really helped relieve the soreness this week.  I know internal work, especially with a female therapist, is off putting to most men, but hell I've whipped my penis out for more people in the last year than I ever thought I would.  At this point it's about healing and getting better.  If that takes a therapists finger up my bum, so be it

I am 3 weeks into therapy and I'd say my soreness is almost completely gone.  I would definitely recommend this type of therapy if you are willing, and have a competent therapist

Treatment: Vit E and fish oil (vitD+E), no more masturbation; good improvement so far, less hourglass both in flaccid and erect state, little to no pain during erection now

4th session today.  No more pain or soreness, she said my pelvic floor is no longer in spasm.  Said maybe one more session and I should be good to go

Hey Tdix,

This is fascinating; I'm happy for you!!

Could you be more specific about how your penis felt/feels now? Did you have any proper pain or was it just soreness? How would you describe the change in perception precisely?

I'm very interested, as I'm hoping pelvic floor therapy to relieve the pain I've been experiencing in erection for two years now!!!
- 35 yo, Peyronies Disease with chronic pain, general hourglassing with girth and length loss since 09/01/17.

This is a great thread, TDix! I'm fascinated by this, and wonder how you found your therapist.  Is she a P.T. who also does pelvic floor work, or is that a whole different specialist.  Wondering what I would look for to find someone where I am (AZ).

My therapist is a PT that mainly does pelvic floor.  My pain was really just soreness.  It's gone now other than when I have sex, and then it's sore for a day or two.  The erection quality and numbness from surgery will take time she says, but the sessions have really helped relax the muscles, as they were all very tight at my first session, and now they have relaxed a lot

I would say that prior to therapy I had constant soreness at my graft site.  I always felt it and it wouldn't go away.  Now after 4 sessions it only shows up after sex, but goes away.

That's awesome, man.  Good on you.  I haven't had any surgery, but have wondering since I got Peyronies Disease 2 years ago if my pelvic floor had something to contribute.

My good friend is a PT, but I don't know that we're GOOD ENOUGH friends for me to have him get up in there and work me over, lol.  But maybe he can recommend a colleague who can.

I hear you Randy.  My therapist is a female and coworker of my wife so I know that awkwardness!  But if they are professional and truly want to help you, it makes it less awkward.  At this point I'll do anything to get better.  Let me know how it goes

I posted this in another thread, but a basic session involves me taking my clothes off and putting on a hospital gown.  She comes in and has me lay on my stomach.  She uses towels to cover me so all that is exposed is my butt.  She then does her internal work for about 1/2 an hour.  She then massages my lower back and buttocks.  I then turn over, and fully covered except for where she is working, massages my nerves on both sides of the groin and scrotum.

Did you just google "Pelvic Floor therapy NYC," or something like that? I guess that's my prob - don't really know where to start.  I asked my PT buddy and he didn't know any of his colleagues who would even knew how to do. He recommended I try for a massage therapist, but that always sounds a little shady....

Congrats man, let us know how it goes. First few sessions you won't notice much, but the therapist should definitely tell you if things are tight down there.  By my third session was when I really felt things were improving.  It appears finding a pelvic floor specialist, especially for men is hard.  My therapist is in high demand because it's basically all she does.  I see her on Friday, so if anyone wants me to ask for any other therapist recommendations you can PM me your location

Started them long before the prostectormy. I think it helped mush.  
Still can't sit on a jar, twist my glutes and unscrew a jar lid  but everything else is working. I have heard of putting heavy washable weighs up your ass helps by tryngti hold then as long as possible.

My therapist actually does not want them strengthened that way, but by relaxing them.  They strengthen themselves once relaxed.  The problem stems from them constantly in spasm as a reaction to pain.  When you have pain in the pelvic area the body unconsciously tightens up as a protective reaction.  Learning a lot from my therapist ;)

Another session down.  She really focused on seeing about my numbness.  She examined my penis in detail and reread my surgery notes.  Said my graft was large and I still have scar tissue, which I knew.  Focused on massaging the muscles specifically related to the penis, and said she may try some soundwave therapy next session to see if it helps.  Usually that is prior to surgery but wants to see if it helps

Very cool.  Are those muscles in the perineum? Upper legs and glutes? I wouldn't even know where to begin with muscles related to the penis.

Went for my first session yesterday. Was really interesting and my PT was incredibly understanding and a great listener. Looks like I have tension in my pelvic muscles that are similar to a knife stab sensation when pressure is applied. Although I don't suffer from ED she said that patients often notice a significant improvement when trigger work is done on certain areas that pertain to the penis/overall pelvic floor. Didn't have any internal work done (that's for next week) but I'm genuinely optimistic going forward. I'll be sure to periodically post my progress over the coming weeks.

(I do want to note that after pressure was applied to a pelvic floor muscle beneath the testes, we did a deep breathing exercise to release the tension. When I was done with this, I went to put my clothes back on and my flaccid penis was completely straight which is something I've seen rarely in weeks)

Another session down.  No more internal work needed as my pelvic floor is now completely relaxed.  No more pain or soreness either.  Did some low dose ultrasound work in the area of my remaining scar tissue to help aid circulation.

Update from my side:
Had my first internal trigger point session today. My PT mentioned something about how the one area of the pelvic floor is connected to the penis and testicles. It was really painful but I managed to get through it with deep breathing. Got up and what do you know? Completely straight flaccid penis. After time it went back to its now-normal semi flopped self but I'm convinced that this therapy is heading me in the right direction. Next session is next Monday, will update then. Happy to answer any questions if the community is curious at all.

That is awesome!  Glad you are getting some relief.  I have my next session tomorrow, and while I don't need anymore internal work, I truly feel it helped me get rid of the soreness and pain.  I'm so happy for you!

Shoot I apologize, I did not.  She works with my wife so I will have my wife ask on Monday.  My floor has been relaxed since about the third session.  She just checked to make sure it still was.

Final therapy session today.  Now it's all about strengthening my pelvic floor with kegel exercises.  Didn't want me to do that at the beginning until the spasms were able to relax.  Now just daily strengthening to get to a strong 10 second contraction.  I'm about at a 6-7, and not that strong, so I have a good couple months to try and get everything strong again

What is the official name/Google category of this type of therapist? If I just search for "pelvic floor therapist" will I necessarily get someone who knows about peyronie's? Sorry if someone already mentioned this.

You have to find a pelvic floor therapist that handles male patients.  My therapist works at the hospital and also fusion wellness.  Their website is fusionwellnesspt.com.  Pelvic floor therapists that treat men are well aware of peyronie's

I had one Pelvic Floor therapy session and she gave me things to work on with my yoga instructor and myself. after abt 4 months Problem solved. Good Luck.

Observation:

I have had problems with my left pelvic floor and hip. Tightness etc. and my peyronies plaques are mostly on the same side.
I can imagine tightness somehow creating inflammation or cutting off proper blood circulation on that side.

The pelvic floor muscles are upstream from the same side of the Penis, the tunica and corpora chambers being split into two sides.
Could be part(!) of the problem and solution.
