How long do "improvements" usually stick around for?

I have been reading this forum obsessively for about a week now. I have come to the conclusion that although some treatments may work a little or a lot, these gains are swiftly taken from you at random times and for no discernible reason. Also that nobody really definitively knows what they are talking about because it affects everyone differently (i.e. endless debates about whether masturbating causes more damage or helps, whether xiaflex works or doesn't, etc.)

How long does any "gain" typically last before it is taken away again?

Is there any such thing as a person who gets back say like 50% curve reduction and it stays that way for 10 years?

I am going to throw the kitchen sink at this but I am freaking out that it is all in vain. I have read for example that dupuytrens cont. progresses indefinitely. So why wouldn't this? I have read about repeated active phases. I can't get my head around a thing that can be fought but not defeated and that has no rules. Rules I can conceptualize. Chaos is a thing that terrifies me. And fighting may make me feel better sometimes but not if anything I do gets crap right back at me.
