Stepone

I am a young 66 year old male.
I am married.
I have had a great life and always had a slight bend in my penis. My penis was 7.5 inches long and 5.5 inches girth.
I would use 50 mg of Viagra and at age 62, I thought this was fine.
In 2014 while having sex, my spouse moved and my penis bent and it hurt like hell.
The next day, when I was masturbating, I noticed on the left side, my penis was sore.
I have not had problems getting an erection, my problem is maintaining one.
Weeks later, my penis started to bend more and there was like a hinge on the left side that wanted to bend when I would try to penetrate.
In September 2015 after meeting with 5 different urologists in 3 different states, I finally found one that said he could help me.
I know it's only 4 years ago, but there was kind of a hush about discussing Peyronies Disease. Guys seldom talk about penis issues.
I had a modified Nesbit surgery in September 2015 and it was a success, except it shortened my penis by 1" and I lost about .5 inches girth.
In the last year, I have noticed more scar tissue forming in my shaft and I am developing an hour glass effect.
I have also been taking 100 mg Viagra and have had to start penal injections of bimix too.
I am up to .90 ML of Bimix. I started at .20 ML in November 2018,
I just recently started taking Cialis too, which seems to be helping.
I have been doing some penis stretching with a device and some pumping, but my penis now measures 6.25 length, below the head, I am 5.25, mid shaft 4.75 and at base 5.50.
I am still able to have good sex twice a week, but the increased dosage for the bimix shots, the further reduction in my penis length and girth due to the Peyronies, and more deforming of my penis has me convinced now is the time to get an implant before I lose any more of my penis.
I am scheduled for a titan implant on 4/25/19 at Duke University.
I will include some photos of my journey.
The second picture is my pre Nesbit surgery.
The first photo is one taken a few days ago.
I will also try to keep a log.

Thank you for posting this update! 4 years is a very long time dealing with such a thing, especially when your symptoms just keep getting worse and worse. Thank you as well for the pictures. Are you doing Ved and traction on the daily? religiously?

Stepone,

Since you are soon to undergo implant surgery I am sure I speak for others in addition to myself about wishing I had a little more info.  Specifically things like how you came to choose your surgeon over Dr. Kramer who is in the same area or some other surgeon.  How did your initial consultation(s) go?  Since many have a keen interest in surgeons, did he measure you?  Did he promise you a post-surgical size, etc?

This is a great time to start your implant journal to track your progress and help thousands of other men with issues they face.

Thanks and good luck
Hawk.

Hawk, I started a post with some pics. I guess I started it in the wrong place..
But to answer some of your questions, more than happy to share.
I had several reasons to choose Dr. Lentz, first closer to where I live, it's 2.5 hours away. Second which is really first, Dr. Kramer no longer takes United Healthcare Advantage Plan. Third is, I read some good reviews on Dr. Lentz and Duke University Hospital and they seemed to fit my criteria.
My initial consultation went very well. I first met with his associate and we discussed my history, previous surgery and they reviewed my records from my previous urologist.
Yes, Dr. Lentz measured and said he could feel how the scar tissue from the Peyronies Disease is holding my penis back from its original length. He said he thought I might get back some of my lost length, but should not lose any. He said the same about girth. I have scar tissue on both sides of my penis where the surgeries were. He said if I truly work at, he would think over time, I should gain.
He also told me that the modified Nesbit that I received, would have reduced my length, based on the degree of the bend. And then the hinge that they repaired was also an issue.
I told him, although I was concerned about losing more length, I wanted my girth back and he said based on what he could feel and see, he saw no problem with that.
If you can find my post from 4/15/19, it will list what I have done.
I agree about tracking my progress. I have gained the courage to move forward with this surgery from reading others posts, including yours. Your posts have been very encouraging.
I am open to any questions I can answer.

Surgery went well. I asked Dr Lentz about early pumping. I said several Peyronie's site members said they stated pumping at 3 days. He said he was familiar with that and actually hD spoken to Dr. Eid.
He said it's a very patient specific length of time. He said it's usually about 2 to 4 weeks.
It's inflated now. Numb and swollen, but doctor Lentz said, they will deflate tomorrow morning around 6am along with Removing the catheter.
He said they should release me about 11ish.
Pain yes, but tolerable.
Duke Hospital staff are superb and nurses very knowledgeable about recovery and pain management. Easy access to Hospital, parking. My spouse is staying at hotel 8 minutes away. Southern Hospitality is perfect.

Stepone

This is day 2 since my surgery. I have been a little spacey due to the drugs, but I remember the doctor saying he deflated me. But yesterday when I looked at it, it was sticking out at 3 on a clock. I read my instructions and it said sometimes it may inflate during the healing process, but rare. Of course the opioids had me confused, but then thought back to my first Nesbit surgery, and it too was engorged, so the clear part of my brain said, wait it's just swollen still.
I have it pouting up in my underwear, but my Scrotum is still sore and now turning black and blue.
I last took a opioid at 3am, and as long as I don't move I am ok. I am going to take a shower after lunch and see how that goes.
Thanks for everyone's support.

It is not likely inflating.  It appears inflated because it is erect from swelling and accumulation of blood and fluid around the cylinders not from saline inside of the cylinders.  It is the nature of the surgery to look inflated for several days to weeks after surgery.  Keep it pointed up as you currently are doing.  You will not feel better every single day.  Sometimes it is a couple of steps forward and then one back but every week should definitely be better than the previous week.

Good luck and keep us updated.

Thanks Hawk!
The meds do make my mind a little cloudy, so  yes you are right. I need to be patient with this.  Today I take a shower, although surprisingly my spouse says I don't stink.
The physician said no scrubbing in the genital areas, just run warm water. I will try to post a few pics, but yes the black and blue is increasing with each day.

I see the doctor again next Tuesday. He asked me if I wanted to start sooner, as each patient is different, but I said No, I can wait.
I have no idea, that was a question I never asked. There is an incision at the top of my ball sack and a very small incision in my groin area on my left side.
Maybe this will help.

Sounds like penoscrotal incision.  It's easier to start pumping if no incision in scrotum...but important still to do.  He'll have more info for you on Tuesday.

Stepone,  Is your scrotal incision horizontal (left to right), or verticle in line with the raphe?

The small incision in the groin area must be an additional incision to place the reservoir.  Dr. Eid does this on rare occasions if someone has had prior abdominal surgery which can make placing the reservoir through the scrotal incision difficult.  Have you had prior abdominal surgery?

Were you instructed to soak lying in hot baths or to avoid the tub?

Dr. Eid typically does a verticle scrotal incision only and pumping within 3 days is very doable.

incision, up and down, best can tell, there is like a sealer over it with stitches.

Yes I did have had abdominal surgery before.

Hot tub yes after 4 days

I can feel the pump, and there is enough room where I can distinguish the pump parts, but I told him I would wait until i have a follow up visit before I start pumping, even though he said I could start sooner.

Mark me down as chicken crap, LOL.

Hey stepone. The sooner you start cycling the better in terms of size outcome and inflation of the device. Are you afraid of the pain you might get?

Stepone

Dam straight I am afraid of more pain and more importantly also screwing things up. I am 66 years old with a few other health issues, I am battling at the same time. This is my second penis surgery. My doc said we are all different in how we heal. I have Peyronie's Disease too, so I am going with what I agreed to do with my doc. I am waiting until next Tuesday when we meet up and take it from there.
Thanks for your input.

Swelling is almost gone, but I am still dealing with an erect penis. I assume this is the way it will be for a while? It does have a crease at the base, when I keep it up against my belly with underwear. When I stand up, it points out at 3pm.
I have a lot of discoloration, which is what my surgeon told me. My spouse said, I thought I ordered it in white, LOL. Love the humor.
My ball sac is mostly sensitive around the incision. I can feel the pump, but am electing to wait to pump, as agreed, until I visit with my doctor.
Since I had small ineffective balls to begin with, hence my need for TRT, the pump is bigger than my balls. My spouse has never had interest in them, LOL.
It is still painful, but nothing like it was, I assume this is normal too?
I have no fever, have not had any change in my temperature since the surgery.
I am checking in to see if my presumptions seem accurate to those who have gone through this and can remember?
Thanks guys
PS. I am soaking in warm baths and also using ice packs on my ball sac, when it swells from walking.

Very good report. Things will improve rapidly from this point. Don't over do exercise or time on your feet. If you spend too much time on your feet following your wife through the stores, remember, ice is a friend. Your testimony once again proves my impression that Nesbit is not a good end all and it does shorten the penis. Cheers man. Keep doing well.

At 2:ooPM today, i will be at 2 weeks. Today is Thursday.
Visited doctor on Tuesday, was inflated to maximum, I saw stars, incision area was burning like hell.
Yesterday was better and I inflated about 3/4 of the way for 20 minutes, so glad the release button worked.
Today I inflated I think to maximum as I couldn't seem to get any more pumps out.
I have had it inflated now for 30 minutes, and pain is just starting in penis.
Should I try to go longer or just keep it to 20 - 30 minutes each day?
The penis does not hurt when I inflate, just the scrotum and pump.
The pump keeps sliding out of my fingers and thumb. It seems less painful when I pull the scrotum skin far above the pump, so its pretty tight. This seems to be the least painful. Suggestions?
I measured and it seems I am about 5" girth directly below head, 5.5 at middle and about 6" at base of penis. I no longer have the indentations about mid length. Yeah!
My old measurements were 5.25, 5.0, and 5.5 for girth.
My length before was 6.5, I am now at 6.0. So this is something I will work on.
The swelling is gone, I have a dog ear on right side near base, so I guess that will eventually go away?
I still have problems sitting, as I get a lot of lower groin pain.

Look forward to comments.

Anything that helps w the pain, such as stretching the skin, is s good thing!  Thanks fir the update.  Sounds like it's going well.  Those who've had the peniscrotal incision can share how long their pain continued (mine was infrapubic).

You seem to be on target with very typical observations.

You can get by with 20 or 30 minutes once a day, but more is better at this stage, especially if your goal is to regain some size.  Getting another 1/2 inch is well within reach, and I would say to be expected if you feel a tight stretching sensation in the penis after you are inflated for 20 minutes or so.  I would try to get in an hour of stretching as a minimum.  You can break that into 2 30 minute sessions.  It is easy to do if you don't have company or kids running around the house.  Pump up before you eat breakfast, shave, brush teeth, etc.  Finish out in the shower where the warm water will help any pain from stretching.  Pump up before bed-time and watch some TV or read.  The discomfort in the penis means you are making progress and getting gains.  Increase as you can tolerate it and find ways to distract yourself, music, other activities, etc.

The dogear will not go away when deflated.  It will get softer without the point, and you won't be aware of it unless you feel around to find it.

If I understood, the pump gets away and shoots out of your hand.  It is not uncommon until you learn to avoid it by adjusting your grip or just stabilizing the pump by resting your other hand lightly against it.  Typically men find that pumping the pump between the first knuckle of the bent index finger and the pad of the thumb works best.  It can help reduce pumping pain if you use a fry soft face cloth between your hand and the scrotum.

It only gets better from here, MUCH BETTER!

Implant on 4/25/19, so it's almost 3 weeks.
Just when I thought I pumped it all the way, I got in another pump. I felt a little pain this time in my penis. I managed to hold it for 30 minutes.
About my pump.
I woke up in pain this morning, it felt like it was pressing up against my cylinders.
I had to take a warm bath and pull gently on the pump to move it around. I took a pain pill too.
Perhaps I am imagining, but it seems like my pump is pulling up to my shaft.
Every day I pump the top of the pump release button it seems to be getting more and more closer to the bottom of my penis.
I think when it first started out, it had more give and it hung a little.
It feels like it is growing out of the underside of my penis like a tree grows from the ground.
I did a lot of stuff, so perhaps I am still swollen down under, as the stitch area is still tender.
There is also a little mass beneath the stitches, did they stitch around the tubing to secure things? I don't know.
Any thoughts?

Good news when fully pumped some pain as your maximally sized.  Hard to say about the placement of the pump.  Mine was also riding high so I kept tugging.  Then one time it pulled loose and is now to low!  Maybe ask the doc to take a look and advise?

Concerning the pumping, I would use two hands to pump the max I could at this stage because the stretch is expansion and your penis will likely adapt to the bigger size.  When you no longer can feel any discomfort or stretch after 30 minutes you are likely the max that your implant will give you.

As far as the pump, if you can pump and deflate, I would not be concerned unless there is pain in the testicles (not scrotum) or unless it looks very conspicuous.

I once asked Dr. Eid if I should tug on the pump to keep it from adhering to the scar tissue.  His response was the most blunt and absolute that I have ever heard from him.  He said, "NO!  the pump will not adhere to the tissue. that is TOTAL BullS**t.  If a pump actually adheres that means there is an infection.

My pump did reposition on its own so I think that is normal.  Also, if it is riding high, is your testicle below the pump or above it?

3 weeks and 3 days from implant.
Ok, thanks for your information.
The pain is diminishing and I finally found some jockey underwear that holds everything together comfortably.
As far as my pump it is riding high, that seems to be where my pain is centered, I massage around that area and gently move it around the best I can, but it feel rooted at the base of my penis.
If my balls are warm, they hang below the pump, but most the time, the pump pushes down on my ballsack below my balls just slightly. LOL, I do have small balls, I think guys with bigger balls probably have less issues. The pump is the biggest thing in my ballsack.
Every day the pump is easier to use and thanks to your suggestion about the two fingers together on one side of the pump and the thumb on the other seem to work much better.
I also started pumping as soon as I get up, before the spouse does, I am able to have about an hour of maximum inflation in the mornings now. Being naked with a fully pumped penis is so much better.
I have also noticed that I can fully pump and a few minutes later, can still manage to get another squeeze or two. Not sure if this means the penis is stretching more? Sometimes i can do this several times.
FYI, I have also gotten to the point, where I no longer feel like, did I make a mistake with this implant. The pain and problems of hiding a painful penis were fairly intense at times, but thanks to the guys on here, my pain was a little more manageable, as I knew there was a light at the end of the tunnel.

After you first time having sex let us know if you have any doubts :)

Your progress is on course and the scrotum is always the main discomfort with a scrotal incision.  It is not a quick 1-week or 2-week sprint as I tell men.   I enjoy your posts and I will definitely enjoy your thoughts as things progress and you have a fully healed penis.

Your scrotum and everything else will toughen up to that "pinched in the zipper" ache and it will no-longer hurt. Contracture after surgery is a known thing to happen after any surgery but with an inflatable device, my opinion, it is something that happens only if you let it and it truly is one of those "man up" things for a little while till everything settles and adjusts. 4 months out you will think the whole thing was no big deal. Cheers man.

34 days since my implant
visited with my surgeon yesterday
he said it looked slightly pumped when I showed it to him
I said, sometimes when I release the pump, it doesn't always release all the way.
He said it is something to get used to over time, it will loosen up.
The doc maximized my penis and said it looked good. He said he believes the Titan Colorplast seems to have a better erection condition than other brands. That is what I have.
However in the unpumped state, they tend to be a little more rigid than other models.
He said after some time, they soften up more in the unpumped state, but really it's the pumped state that is the most important, so most of his clients prefer the Colorplast.
After the inflate, he pressed the release and squeezed the penis until it was a little sore, he said it takes a while to get most the fluid pumped out in the first months.
He told me to come back in 3 months for a follow up and that he thought I could use it, once I was ready psychologically and physiologically. I said great and got a prescription for the testosterone that I needed refilled and left.

My penis was a little sore from the squeeze and the ride home, so even though I thought it would be a good night to give the guy a test drive, I decided I just wasn't there last night, and the spouse understood.
I hate to say it, but I am a little nervous.
So I am thinking about tonight. I still have pain in my scrotum from the doctor pumping and releasing, but would still like to test the thing out tonight.
If I do, I will certainly post, but in the meantime, wish me well.

Hey Stepone! Your doctor is right, about the rigidity of the coloplast titan versus the others. There was a pdf article i saw from Dr. Eid a while back, where he tested the bend rigidity of the Titan VS the AMS and it was impressive how much the second brand flexed even when fully inflated. You also have a couple of members that had the same implant here before you and are almost  at their 1 year mark and you can make a comparison with them.

From what i read, the cylinders do deflate better as time goes by, and also get softer. Dr. Eid also has some videos of his patients deflating the penis the same way your doctor deflated yours.

Grab yourself the courage and take it easy.... you deserve it! Best of luck on reentry! :D

SW01

It definitely gets better over time. Going on 3 months here. Cylinders are definitely softer but I still project out pretty good flaccid. Probably about a month ago it started to just deflate further. Happened kind of spontaneously. Now when I push deflate button and squeeze penis I can see the folds. I could not really before. I am just squeezing out most of the saline and leave it hanging around 4.30 or so.

My only issue is I am still tender in the tips. I am still pumping up daily and sometimes a couple times a day.

Positive is my peyronies curve is noticeably better. After implant i had a very slight curve left over from my 30 or so degree curve. Hardly even can tell now. My right tip was a little lower than my left one initially. It was still in the glans well though. Doc said as curve straightened they would even out. Slowly happening.

Long race here to get back to some semblance of normal. Happy where I am at 3 months so far. Hope things keep getting better and better.

I am so happy this worked, my penis head was a little tender and still is, but Gosh, sex was great.
The pressure is off!

Yeahaw!!!!

That says it all.  That is the very essence of an implant.  Your life no longer revolves around; if you can have sex, what planning you have to do to make it work, the constant almost subconscious concern with your limitations.   G O N E  !

Congratulations.  It only gets better from here on!

So I guess it's about 45 days. I still have a sore ball sack sometimes and my penis is still a little sore sometimes. I still have about 3 stitches that refuse to disappear. I also have a tender area at the top of my ball sac. It's like a mass of scar tissue.
I have been massaging my balls, the scrotum and pump as my urologist said when I take a shower, I should be moving them around a little to keep scar tissue from locking up. I think that's what he said.
My penis continues to pump up nicely twice a day. Sometimes it seems I can get a few extra pumps and yes things get very hard. If I max it out, at about 30 minutes, I will start getting some fairly intense pain, so I know it's for a good cause, increasing my size, so I bare it.
I have had two intense sex F~@ks with the spouse and hopefully tomorrow night too. I would want more, but twice a week works for me now.
My spouse is loving the new me and my new penis.
I never thought I could have such a hard penis again and increased girth and pleasure.
I questioned my decision a few times, but each day brings less pain, so I just had to get through it.
I look forward to getting to the 6 month and then a year.
I also haven't found a pair of underwear i can wear all day. I am wearing a jockey blend brief, it seems to be the best at keeping my penis mostly upright, but my balls and pump can get a little too tight and I get some pain.
I cant wear boxer briefs as my penis refuses to behave and it attracts too much attention.
I am also being patient with the semi erect penis I seem to constantly have, but it is becoming more pliable and the two dog ears hurt less.
I am very happy with my new bionic tool and look forward to when it becomes more a part of the rest of my body.
Thanks again to everyone's support and help.

3 of my internal dissolvable suture took over 6 months to disappear.  Hang in there, like you said it will all be worth it!

I am now at 60 days with a titan implant.
In retrospect its been a tough road, but I finally am on the mends.
One note is that some of us experience pain differently. I tend to handle pain fairly well, but when it impacts my sleep I go into deep despair.
I still sometimes will use a beaker in my bathroom to urinate in, because I don't feel like bending my penis down to get the urine in the toilet. I sometimes wish I had a urinal in my home, they are so easy to use, LOL.
I can sit a lot longer than before on a hard chair, before it was painful for my scrotum. This has just recently improved.
I have noticed to that the pump which felt like it was been sucked up into my penis, has finally relaxed and is more moveable in my scrotum.
My pump is bigger than my balls, I always had small balls, but now I almost feel I need two pumps and at least I would have a matching set, LOL
The pump is so much easier to use.
Special thanks again to Hawk, he has given me so many helpful hints. My top three are....
1. when I get up in the morning and have my coffee, I pump up, read my emails. This is my new routine.
2. to learn the pump better, repeat the hand movements several time to cause the hand muscles, etc. to teach the brain, this has been another great tip.
3. be patient
Oh and I finally saw the last payment go towards this surgery. My total bill was $73,463! What? Yes its true, I spent the night at the hospital and was discharged the next day about 3:30pm. I must say that Duke hospital took excellent care of me, quiet, responsive, and very kind. My share of the entire procedure came to about $1500 with some extra lab work, etc.
But getting back to this journey, I am so glad I finally took the leap to get this implant.
I no longer have to worry about injections, pills, that were no longer working.
I could get an erection, but it wouldn't last.
Now I pump it up and are ready to go, each time I pump it gets easier.
I still experience some tenderness if I have it pumped too long, and even some pain, but it gets less and less.
Earlier this week, I had the best sex I have had since 2014.
I still have numbness on the bottom side of my penis, which sometimes hinders my climax, but I hope this will improve.
I have never been a quick shooter and I sometimes wear out the spouse, but am thankful that patience is one of their strong skill sets. LOL.
I have chatted with some guys that are quick shooters and their spouse say, they want them to last longer.
Mine wishes I was quicker, LOL.
So back to the implant.
I still am trying to find the best way to keep my penis in my pants, but not bulging so much, I get everyones attention. Some days are better than others, but it's not such a "hard" disguise as it originally was.
I walk around naked sometimes inside the house and this is the most comfortable thing I can do, I think it has also allowed my penis to start hanging more.
I am back to the gym which has improved my self confidence and I am trying to keep in good shape for those marathon sex days, LOL.
I hope to post a pic soon of the new improved penis, but I need some alone time and I will send one out.
All is all, glad I started this journey and so thankful for all the helpful guys on this site.
....and as usual, special thanks for Hawk for being here for so many of us.

As I've just said in a reply to you on my own journal, Stepone, thanks so much for all YOUR support too. Reading your journal has been inspirational for me. You are 37 days ahead of me in your recovery so keeping up with your experiences has been really helpful to me. You're doing great and must love the fact that you had the courage to go through it all. $73,463 tho?? Wow!! Thank God for the NHS over here in the UK!

Take care and keep posting your progress please.

Yep. My total bill was also in the 70,000 range with an observation night in hospital. My cost in copays was around 900 or so.

Not sure what it would be if you paid it yourself, but I would have to know up front the cost before surgery.
