27yo, still trying to figure out if it is Peyronies

Hello everyone! I found this forum recently and find it impressing how people help each other here! I am 27 years old and still trying to figure out if I have peyronies disease or - hopefully - just something rather harmless. I will try to make it short, but of course that's difficult when talking about one's medical history...

About 2 months ago I did something stupid - While masturbating I hurt my penis by bending it downwards. There was no bad swelling, no popping sound or anything, but I could tell that something was wrong because it became extraordinarily flaccid very quickly. I should note that something similar had already happened to me 2 years before (yeah, no learning from mistakes) and at that time everything healed perfectly fine within a few months. Because of some minor pain on the right side I went to see several urologists anyway who checked me by palpation and ultrasound. No plaque was found and I was told that there was nothing to worry about.

So I just stopped any sexual activity for a few weeks, which was easy as I am in a long-distance relationship. When I had sex for the first time after a month or so, I was a little in pain, but not bad enough to worry about it. After 6 weeks I started masturbating regularly again, when I suddenly noticed that my penis was curved to the left maybe about 20 degrees when erect and generally dangling more to the left than before. My penis is naturally bent downwards a little, which was never a problem for me, but the new curvature was a shock. Of course my first thought was that I had given up the sexual abstinence too early. I'm still not sure if this was a coincidence or not and unfortunately I don't remember when exactly I noticed the curvature for the first time.

I still couldn't find any plaque and neither could the urologist who I went to see again. What I also found strange was that the curvature was to the left, whereas the pain had always been on the right-hand side and this is where you would expect plaque to appear first, right?

Actually I am not sure if the curvature is really in the cavernous bodies, because to me it kind of seems as if the whole penis was tilted a little and now the curvature that used to point downwards is more to the left. Unlike the typical injuries in cowgirl position - where the penis is probably bent somewhere right in the middle - in my case the biggest impact seemed to be on the penile root (where I also feel slight pain until today). I also noticed a weird dimple slightly to the right of the penile root. So I thought that maybe the suspensory ligament of the penis was damaged, which would explain the symptoms perfectly, but couldn't find any information about such injuries online...

Of course I am worried that it might still be peyronies. Someone in this forum wrote that the absence of plaque in this early stadium doesn't necessarily mean that it's not peyronies. However, the urologists don't seem to take the problem seriously unless they find any clear bumps. Now I don't really know if I should do something just in case or if I should just wait and hope that everything heals back to normal...

Still no response, so apparently my case is so vague that nobody knows how to help me... Really any suggestion would be helpful! It's the uncertainty that is especially hard to cope with currently.

In the last days I have been suffering from strong pain on the right side, feels like an inflammation. Everytime I touch it to check it for plaque the pain gets worse. I still can't find any specific bumps, rather the whole right cavernous body feels hardened. The curvature didn't progress, though.

Maybe someone can help me with some very specific questions:

  • Can an ordinary person actually detect plaque without any doubt if there is any? Or is it possible that I am just not sure about it because it hasn't fully developed yet?
  • Can there also be ordinary injuries of the penis with similar symptoms (inflammation, slight curvature) that don't result in Peyronies? Like, if you don't have the genetic predisposition? Unfortunately it's almost impossible to find any information about it because most search engine results lead either to penile fracture or to Peyronies.
  • Does the fact that I had a similar injury two years ago and it didn't result in Peyronies speak for me to not be prone to Peyronies, or can it also emerge after several incidences without any complications?

I would find a peyroines specialist willing to do an ultrasound.  Plaque cannot always be felt.  You may also want to ask for the ultrasound that checks for blood flow of the erect penis to diagnose any reasons for ED.  It sounds like it very well could be peyroines w additional problems as well.  Take care and keep us posted.

I agree with Tsan. If you don't have an ultrasound, you really don't know what you are dealing with. As for feeling the plaque, I have never really been able to feel mine. However, what I can feel or notice is a flatness in the shaft where the plaque is, but this palpable plaque is not something that I have ever noticed. You won't necessarily have visible plaque right off the bat.

I think to your 3rd question, peyronies can be tame for a bit but further injury aggravates it. Like when first had a problem I had sexual experience too soon and may have made it worse. I think I have more scarring as a result than I would have if I'd been a monk right after initial injury. And I hadn't had sex in a while so was tempted. Timing is never right, right?

I had similar masturbating injury with cockring. Went flaccid real fast, like I popped a vein or blood vessel or something because right on top of penis right about an inch south of my glans had a lump. Swelling seemed to subside a little but never was the same I don't think . 3 drs (including 2 uros) said they felt nothing. But when really flaccid I think the residual lump is still visible. It's not usually painful (except if I jerk off too much or lean in the wrong way while sitting, like putting all my pressure on it. Then recently I think I injured glans by ejaculating (like my urethra Is bent at an angle) and cum shoots out sometimes hitting inside my penis and oozes out). This heals and recurs if not careful. Can feel a little bump on the head but if I don't cum for a few weeks bump smoothes out with the rest of the skin and cum shoots out again. These are two different issues and is relatively minor.  I say all this because we all have such unique stuff going on, and sometimes 2 separate problems can conflate and react together. Fortunately it can always be worse.

You definitely need an ultrasound like everyone suggests. I've put off following up with docs since I was demoralized by their inability to diagnose and "fix me" as soon as I presented to them.  I also lost job with insurance but that's no longer the case now so I can go back to dr. Just waiting to see what I can do on my own and monitor for 6 mos.  But be persistent and try to abstain from masturbation for a while if possible. Sometimes this really helps and gives penis time to heal naturally. I ordered pentox online and thinking of trying it once I quit cigs. Just don't know if I want to take a long term pill with side effects with no improvement. I want to try VED, stretching and maybe pentox / low dose cialis. Just don't want to mess my body up any worse since my penis is very usable now-- just not as good as it once was. But who wants to settle if one can get healed.  Supplements can help a bit tho for overall penis health.  Getting a healthy diet and exercise (and sleep) can also be beneficial.  Best of luck to you my friend. The longer you wait a proper diagnosis, the more likely it is to get worse.  Consider starting the scar minimizing remediation things immediately...various supplements discussed here...including getting a good Magnesium supplement going.  Your penis is injured, and will "heal".  Some scarring is likely...and you want to minimize this as much as possible.

It is a good thing to become erect fully daily if that is possible, as long as you don't aggravate the injury further.  What you don't want to have happen is scar tissue to settle impairing the function of the Tunica further.  This is somewhat akin to flexing a knee after surgery so that it doesn't "freeze".

As I mentioned in my initial post I have already had an ultrasound right after the injury happened, but of course it's possible that at the time the scarring hadn't developed enough to see anything, or - what I feel is more likely - that I have worsened the initial injury later when becoming sexually active too soon.

Yesterday I went to see my regular urologist again. He took the time to check me for bumps and did an ultrasound, but couldn't find anything and didn't seem to see any cause for an inflammation. He was patient and polite, but I could tell that he thought that my pain was mostly psychosomatic and that I was worrying unnecessarily because of terrible things I had read on the internet. So although he couldn't give me any alternative explanation of my symptoms he suggested that I shouldn't think about it too much and go back to a normal sexual life, because in the worst case it might be painful, but nothing more. He also advised me against taking any anti-inflammoratory drugs.

I don't know, it's perfectly possible that my injury is harmless and I am making it worse by getting worked up about it too much. However, what irritated me very much is that he said a few things about Peyronies disease which according to what I have read here and on other sites are plain wrong:

  • He claimed that in case of possible Peyronies Disease you usually wait and see in the beginning and don't start any treatment before there is any plaque
  • He claimed that in cases of Peyronies Disease spontaneous remission is quite common
Can anyone confirm that this is bs?

So for now I will try to follow his advice and not worry too much as long as it doesn't get any worse, but I will definetly not just start to masturbate again as long as the pain is so bad because I am sure that his perception that it won't do any harm is wrong.

He didn't even mention it. He seems to have no clue what's going on, he is only convinced that it is harmless.

When I told him about the curvature on my last visit he just responded that some curvature is very common and normal and that it doesn't need to be treated as long as intercourse is still possible (as if I wasn't already aware of that).

But like I said in my initial post, I'm not even sure if the curvature is new, or if it's just my natural curvature which now points more to the left because the whole thing is now a bit tilted to the left. (Remember also that my pain is on the right-hand side.) I would have liked to discuss this possibility with him, but he is one of those physicians who mostly talk and hardly ever listen...

You mean another urologist? I've already been to two other doctors immediately after the initial injury, plus one at a walk-in clinc (the latter was a complete waste of time). Only one gave a competent impression and listened carefully. I might go see him again in a few weeks if I feel like it's not getting better, although he already criticized me for seeing so many doctors on my last visit. But maybe he will try to answer my questions about the curvature.

If you're talking about a Peyronies Disease expert, I'm afraid there isn't any nearby.

Like I said, I don't know. It definetly looks different from before, but it doesn't seem as if the penis curves around some scarred tissue, because then it should be on the other side. There are for sure many other ways to end up with some curvature that you were not born with (some people are even convinced that masturbation in their youth had an influence on it).

I'm not a health professional, so I can only take guesses. Maybe you can get a torn ligament in your groin area by bending the penis the way I did, leading to one of the ligaments being a bit longer than the other. If that is the case it might take a long time to heal, but the curvature might also be reversed eventually. Or maybe the right cavernous body is a bit torn because of tiny injuries.

Are there any people in this forum who have had other diagnoses than Peyronies Disease and might have an explanation?
