Hi I'm a 35 year old male and last year I saw a urologist because I was feeling pain at the base of my penis after an evening of rough intercourse, he advised me to take it easy during intercourse and avoid certain positions. He also advised me that if my pain worsens to come back for further treatment. The doctor told me I have acute peyronies and that it should heal on it's own after a few months. It did get better and then I reinjured it again during intercourse. But now the pain comes with an erection. Some days it hurts and some days it doesn't, it all depends on the position I am having intercourse in. I still get erections and my penis is still functional. I've been reading a lot and seen pictures and I am 50/50 on doing the viaflex injections. Both my doctor and I felt the plaque/calcium buildup on the base of my penis. But the difference between what I am reading on this forum is some guys with peyronis have a significant bend and have to mold their penis' after injections. My penis isn't bent, it just leans a little to the left and occasionally painful during intercourse and sometimes during erections. I'm just afraid to get this shot and I am wondering if it is really necessary.

Someone please help with some advice. Again, I'm not experiencing any deforming or bending of my penis, it just leans and there is occasional pain at the base. Thanks and I apologize in advance if I sound ignorant to this.

First Timer,

Has your URO done a doppler ultrasound? If you haven't had an ultrasound exam, how do you know you have Peyronies. If you had a curve, it would be highly likely, but without confirmation through ultrasound, you are just guessing. Having gone through 4 cycles and about to start a 5th, you need to have an ultrasound exam before you go through Xiaflex. If your URO wants to do Xiaflex without ultrasound measurements, run away and find somebody else. How would he know the length, width, and depth of the plaque? How would he know where he needs to inject?

I would NOT use Xiaflex unless you have a real deformity caused by an obvious plaque. It isn't any simple shot.

Consider all the supplements, drugs, and physical therapies as soon as possible and stop having sex. Leave your penis alone for a week and avoid all erection checking. Take an anti-inflammatory if you like, though I recommend beginning CoQ10 at 200 - 400 mg a day immediately. How much time have you rested since the injury? It can take a very long time to feel alright. I recommend abstinence and traction with supplements and or pentox / cialis. Of course you can have sex or masturbate if your body needs it, but penile injuries are serious. Anyone who tells you to persist and keep having sex is insane to me.

The penis doesn't need to heal. You have to understand this. There is no genetic reason for the human penis to heal, because if an early human ever was able to damage his penis, he likely ejaculated and passed on his genes anyway. When you injure your penis, you need to take it extremely seriously. You are NOT going to be okay by doing nothing or relying on Xiaflex or surgeries. I know you are taking it seriously, but I'm just trying to hammer home the message. Any penile damage puts you in no-man's land. Very few urologists know what their doing. Do your research and learn everything you can extensively before deciding on Xiaflex. Consider pentox and cialis if you want to take drugs for it, and CoQ10, ALCAR, Citruline, or other things on the supplement side. Look into traction immediately and VED if you have denting. Heat is also an option but right now you may just need rest. Inflammation stays trapped in the penis for months.

I would also say wait on the Xiaflex, but consider the oral medications.Xiaflex is approved as a collagenase and is used to break down collagen plaques that form and cause curvature of the penis.Be careful with injuring yourself further or even permanently.
