grafting top of shaft is trickier to maximise length gains

for more severe curvature, complete straightening might be limited by the length of the neurovascular bundle lying on top, which makes sense the top is shorter and bottom longer. stretching it too much will cause nerve damage. some surgeons will instead also do a concurrent nesbit procedure to the longer bottom side. make sure you are informed about this, you can request not to do the additional nesbit and be left with a slight upwards curvature which is fine anyway

on the other hand grafting the bottom part is no problem since the urethral vessels are elastic in nature

I don't follow your logic. Maximizing length is just as tricky whether the curve is up, down, left, or right. Surgery with an upward curve is tricky simply due to the location of the neurological bundle. This is why you only want a good experienced surgeon performing this kind of surgery to avoid damage to this.

I had a 70 degree acute upward curve that was completely corrected. There was no Nesbit involved.

I think it applies more if there is fibrotic changes to the neurovascular bundle which shortens it, which will definitely limit how much the dorsal tunica can be lengthened without placing stretch on it. YMMV and its something that can be only determined once on the operating table. Anyway, a less than 15 degree residual curvature is mostly deemed a good functional straight result, but some surgeons *might* correct it with a concurrent contra-side nesbit. Does it make more sense now?

Seems like you have adequate 'redundent' NB bundle slack which was unaffected by your acute plague/fibrotic formation.

Well guys my surgery is this Thursday and i'm rather nervous but i know I have one of the best URO's on Ontario doing the surgery. Not sure if its proper or if I need permission but I would like like to post some before, and after pics to document my recovery etc. I hope by doing this it may help my brothers when deciding about surgery options etc. If any admin would post or reply I will follow the procedure.
