Hi there,

as I finally got my diagnosis last week, I'm doing my introduction now:

Symptoms: Curve to the right (25 to 30°), upward (5°), hourglassing on right side, some distance below the glans. Related: Chronic Pelvic Pain Syndrome

I'm in my mid-30s. Early 2016 I suffered an injury during intercourse with my SO in the famous cowgirl position. Basically hitting her pelvis on the downward motion with 90% stiffness. No loss of erection, but couldn't finish due to pain and shock. Pain was gone the next days and I was optimistic I would get away with it. No bruising.
Some weeks later I noticed an indentation on the lower right side, followed by a small curvature and missing nighttime erections. So I went to my urologist. He did a clinical examination (just feeling for plaques) and told me I most likely have Peyronie's. He could prescribe me "a handful of pills", but in the end it wouldn't matter and I should wait and see.

After a few weeks I decided I would rather take a bucket of pills than doing nothing and regretting later. So I informed myself about the condition and found this forum, worked out a supplement route to go. Went to my GP and told him I'd like to get Cialis prescribed for improved blood flow. He was very open about treatment and did supply me with the prescription. Indentation got worse, but at least the pain got better. In the meantime I tried getting a second opinion of another urologist. He couldn't feel any plaques and told me to just get a surgery or live with it. I noped out of the surgery route for now, as the risk / benefit ratio is just not worth it at the moment.

Until that point, no urologist would do a Doppler ultrasound. Talked to my GP about that and he said he couldn't do it in his office, as he does not have the necessary ultrasound head. So I researched further and found a clinic in my county with several urologists. That was actually the first urologist I've ever met that was friendly and competent. He did a clinical examination again, told me that it is obvious I have some hardened tissue and scheduled an ultrasound.

So last week I finally got my Doppler. He induced an artificial erection with an injection which didn't fully expand the penis, as I was lying on my back (having some problems with that), but he said it would suffice.
Left side (healthy): Blood flow of the penile artery was basically of the charts. 30 to 35 cm per beat velocity. The device was clipping the signal at 25cm per beat.
Right side (Peyronie's): No beat detectable on primary artery. No calcification. Thickened tunica in area of indentation (wasn't told measurement, but I guess around .5 to 1mm thicker from seeing the images).

His diagnosis was the thickened tunica is compressing the blood flow of the right artery, as he could not find direct damage to it. This leads to delayed erection, curvature during erection, while the inflexible tunica is causing the bend and hourglassing. The right corpus is basically filled with blood from the left side as there are many small blood vessels in between.

Therapy for now: VED (prescribed), further Cialis and supplements. If it get's worse over time, surgery is an option, but he would refer me to an urologist who is specialised in this kind of surgeries and will choose the right method for my case. The right side is actually pain-free most days, with the left side hurting due to the bend on erections, as the outside is getting stretched from the curvature. I will get my VED device soon and maybe consider traction. No calcification is imho a good sign, as the tissue is still malleable.

My stack as of now:
2.5mg Cialis
2000mg Acetyl-L-Carnitine
400mg CoQ10 (started recently)
1000-2000mg L-Arginine
500mg Bromelain
For pain: Apply topical Diclofenac / Voltaren, which usually helps after a few minutes and should also counter any inflammation.

Lessons learned: Don't let bad urologists get you down. The whole condition is depressing and these guys will further demotivate you. Seek a doctor with good reviews, and if he just is mumbling nonsense about "wait and see", move on to the next one.
Also: Having to walk half a mile with an erection in tight jeans is NOT fun. Park your car next to the doctor's office and get comfy trousers for any examination requiring an injection  ;D
