Scar tissue at the base/internal penis
« on: February 04, 2017, 10:26:54 AM »

I've read everything on this forum about curvature and plaque on the shaft but cannot find ample info from people suffering
from scar tissue at the extreme base/internal penis... Is there anyone on this forum that has this issue
which results from blocking blood flow and shrinkage of the unit?

I would really like to correspond with you guys and see what has worked and not worked on your journey.

Thanks!

Scar tissue typically forms on the Tunica. This really doesn't block blood flow as the arteries feeding the penis are deeper in the Cavernosa.

LWillisjr,

Thank you very much for that. That does make sense. Im trying to get to the bottom of why my penis is
shriveling and losing girth.  I had inguinal hernia surgery earlier this year. That coupled with Peyronies might
be the culprit. a lot of scar tissue.

Anything else you can an add would be greatly appreciated.

Thank you, LWillisjr.

the urologist at the VA stated that I had scar tissue in between the two Corpus Cavernous which could be causing lack of blood flow. The other urologist had a difficult time finding the scar tissue. That doctor did notice hardening at the base. Hoping to get an ultrasound soon.
thanks for the help.

I re-read your posts. I am very skeptical about one doctor's physical examination and saying you have Peyronies. I believe an ultrasound is much more accurate in the diagnosis of Peyronies. So my humble opinion is I am hesitate to agree if your have Peyronies.

That being said, I do believe your symptoms could be related to the hernia surgery. My advice is to always focus on the symptoms, and in your case that seems to be loss of girth. To help with girth loss, I would suggest you read/study the section on the forum dealing with VED's. Careful use of VED therapy is what i would do in your position.

Thank you, LWillisjr,

I hope you are right. That is very good advice. I will focus on the symptoms and will push for ultrasound
thank you again.

LWillisjr,

Its been about 17 months since my injury and Ive been taking 400mg Pentox 3x day, CoQ10, L-Citurlline, cialis, fish oil everyday and despite my efforts my problem has progressed.  Narrowing at the base and seems to might be the beginning of buckling.  Ive had 2 doppler ultrasounds and still the dr (Dr Gelbard) said there is good blood flow but with venus leakage. no sign of plaque.  I've done PRP (sham) and flirted VED but it hurts even with the slightest pump. When I do pump my penis balloons at the base and doesn't fill the glands or top so I know there is plaque encircling my base. Not a usual case of peyronies but there is some peyronies or maybe even corporal fibrosis.  Everything is self diagnosis. these doctors are no real help.

So Im wondering what to expect in the future. Continual progression until ED? continual shrinkage? when does it stop!?
Im at the end of my rope.
Any suggestions would be greatly appreciated.

patman,
I also have a Plaque , running circumcidiär araund the base at the very bottom of my penis.
It starts 15 Month ago with a little small node at the right side of the base - developed since then around the base in direction to the left side, were it finally arrives the oppsite side of the base.
During VED use, i can also see this "balloons at the base" Effekt.
The different seemed to be that i can clearly feel the Plaque and that i was able to easily measure its progress during the last Month.
So far as i understand it , you are actually not diagnosed with pyronies?

Concerning Venous leakage:
It is not necessarly a Venous leakage, but maybe to many kollagen instead of smooth muscle in your penis.
I have also good bllod flow which seemd to flow out too fast because of that issue with to many Kollagen in comarison to smooth muscel (the optimim should be 55% smooth muscle / 45% Kollagen).
My Dr. put me on Cialis daily (5mg) , VED and Sildenafil (50 mg prior too sleep) , to overcome this problem.

Yes Dr.'s are no REAL help, i visted many of them and still not over to find others.

But in your case its realy important to determine the cause of your problem , all other aspekts are speculations that dont do anything than lower your mental state , which is also an important Aspekt to fight this F... Desease.

So you may have to find a specialist first to examine exactly what your issue is about.

You mentioned that you are 17 Month on pentox and so on - You began to trat your self prior to your  inguinal hernia and you mentioned an injury.
Is your pyronies confirmed ?

Kovasis,
Thank you for the response. Ive been on pentox for a year when i was given it by a urologist at the VA. But he said he thought he could feel scar tissue but was. Ot sure. Since then ive seen several other uros and they feel nothing and one said it was all my mind.

So ive never gotten a true 100% diagnosis. Just speculation. Everything is mainly my research and doing from info on this sight.

Does the VED help you? Does the balloon effect go away?

Do you experience shrinkage especially at flaccid state?

It seems our problem is in the minority. Very frustrating

Wonder if i should just get ready for an implant before it shrinks more... but id still have the problem with the contracting scar tissue around my base.

Thanks for your help

Patman,
As bald as it may sounds but at very first you need a 100% diagnosis, or at least an exact determination of what this issue , your suffering is about.
Even so called specialists are sometimes not sure ,  but in my case at least i now there is a fibrosing formation at the very base of the Schaft and that it might has developed there due to a Micro Trauma.
I had an accident in my late teens, that left a little scar (0.5 mm (discovered  via MRT)), now it seemed that the fibrsois formation is a kind of progression from that almost 30 Years Old Scar due to that above mentioned Micro Trauma.
Call it Peyronies Disease Call it Fibrosis - its all the same and needs to be treated as such.

I did not mesaure my lenghts prior to Peyronies Disease , but do measure it since almost one Year now. It did not loose any lenghts since measuring but it seemed shorter now in comparison to prior Peyronies Disease (this is just a guess, a subjective impression).

So i would say maybe about 1.5 cm loss of lenghts...

I do mesaure always in Erected state , because flaccid state is anyway differently, depending on temperature and so on.

VED doesnt help me and i think (in my case) it also weakened my NTE (Night Time Erections) and leads to kind of discomford every after i used it...

I managed to avoid the "balloon Effekt" by using a smaler lid (which was delivered along with my VED Cylinder).

So inbetween i only use it to pump maybe 30% , in a way that only the Top will be inflated, so far as possible.
prior to that i used to Pump to 100%.

VED is a good option , but not for everyone. You have to figure out if it works for you, very carefull and with many patient...

Can you explain what you mean by "contracting scar tissue" ? .

As i mentioned , my fibrosis (buckle) at the very base is palpable (in flaccid state) and during Erection state it is hard to examine (but of course still there) ,  seemed to expand with the tunica, so that one can only feel the weird surface of it (very small , little dots).

I'am still playing also with the thought of having an Implant , before evrything may get worse, but they told me that even that solution cannot be done before you reach the so called stable desaese state, because otherwise it would lead to more damage after operation.

When ever that state will be reached...

I dont believe in the 18 Month Theroy , simply because a desaese does not have a timer to countdown... espacially not this desaese.

Recently i was told that it could stay  15 Years (as it is) in a kind of plateau ( no progression) before it proceeds, or in very rare cases it may vanish by itself within 2 Years...
Since i never heard bevore from the 15 Years Plateau , i asked him (the URO) if he read this information in a fortune cookie or something :-).

So just dont believe anything they say...

Every is case is different and so its very important that you find at least one "Expert" who examines what kind of issue (I wouldnt call this pyronies as long as it is not conformed) you are really suffering.

That is not true  the best implant surgeons say the opposite to this (Perito, Eid, Kramer) They say that it is better to do the implant as soon as possible, so the plaque will not be calcified and will be easier to insert and you will not lose any more size

suicidecomingsoon,
Thank you for your input.
I just ask the prof. who examined me in Mai This Year , if it would'nt be better to head for the Implant solution, rather than wait until things may get worse an thus will even complicate that "solution".

On his Opinion , any operative investigation in the so called accute / unstable disease state may lead to rezidiv / new injury and inflammatation as well.
This sounds logical to me.

But i honestly do not see any logic in this whole disease, so who knows?

I cant see how will one ever be sure in what kind of state one is?
The pain may subsid , the plaque or what ever may not grow - but how will one ever be sure to not have further progression due to other injuries or minor Trauma?
How will one ever know that it will not progress after lets say 3 or 5 Years?

If there were something like a Test (Western Blood or something) which would exactly determine if there are lets say inflammations marker that tells you if you are in the accute Phase or not, it would be more confident...

But so far as i know there exists nothing like that and so i think one have to doubt if there are something like accute and stable at all.

next month i have another appointment , where i will discuss this again in more detail with the Doc to whom the prof. referred me .

I will keep you informed

Kovasis I read that Dr perito says
What do you consider a satisfying result from non-surgical Peyronie’s treatment?

Because I am the implantor, the patient must be perfectly straight when completed. I also push them quickly to surgery in order to avoid more loss of length."

ALSO

"How do you address the emotional side of Peyronie’s? What can men do to deal with the distress caused by the disease?

Early implantation limits the loss in penile length."

It is assumed that the implant stops the progression of peyronies disease, say the best surgeons. I read it in this thread Implant in the acute/active stage - Peyronies Society Forums

What a great discussion, thank you both for the input and the time.
Yes, everyone has a varying opinion especially with implants. I agree with the eid, kramer idea because if over 50% progress and to kovasis's point there is no time table. I dont believe the 18-21 month rule. Maybe it will stabilize and then 2 years later start to progress. Im 37 so i assume this evil disease has plenty of time to preogress... implant does make sense but you have to remember as well that surgeons only make money when they "cut". But is hard to take that big leap of turning to an implant. A huge change in life.

No one knows.

What I meant by constriction kovosis is that i feel the scar tissue is constricting my base and making it tight. Thinner and possibly restricting blood flow, hence my shrinkage especially flaccid.
I almost feel as though blood is "backed up" and unable to fully enter my penis especially when sitting a long time. That would be a reason for cooler glands and shrinking head.
Ive just tried to introduce blood into my penis as much as possible.

I wish some of these so called doctors had this disease then they wouldnt be so flippant with their opinioms and lack of seriousness to us.
Thanks

suicidecomingsoon, Patman
Yes, it is assumed... But this is the F... Shi... about this disease . It may work for one and do not for the other.
I'm almost at the point where i dont believe anything of what they tell us. The only thinng for sure is that it is not sure.

I guess they simply dont know themselves , dont really care or not really interested until it comes to a lot of money to pay for what ever they prey to you , will help you.

The sad trues is that at the end of the day we are sitting here, day by day and hope that some miracle will heal us.
It may be helpful to talk to each other or to report the one or the other  success or unsuccessful treatment, at least  to cope somehow mentally with that F... D....
But to be honest, all that stuff, all the Storys are more anecdotal than really helpful and manny of them even depress people more than they were already depressed bevore (me included).
This is why i Think that NO ONE Really knows what it is about and that we have to swim in an Ocean of unknown and ignorants until we maybe see an ireland to land on...

Yes, i will proceed to swallow those F... Pills and I will proceed to try this and That and maybe in the End the Implant "solution" ...
But what really scares me is that nowadays there are so many men suffering the hell and nothing really happends...

I wish you both also to hang in and try your best , but i would stay very suspicious especially when manny Money is in the game.

As I mentioned earlier.
Try as exact as possible to discover what is really going on down there and then decide what you maybe do next

To the original poster, pm me as I have plaque near the base. I had lost nearly 2", girth and had erection issues. I have had my first cycle of xiaflex and am using traction.

I also have a plaque just at the base and i think it made it smaller by at least 1 cm and i have much smaller girth as well. It is not seen in pictures so even though i tell the doctors they don't believe me or believe what i say. It is like every doctor and people in the world tries to make us suicide, rather than help us or solve the main problems of this disease.
