The idea that I'd wake up one morning with an erection and find my penis broken was never in my consciousness. Yet apparently it's very common.

Anyway, this is the story of a successful treatment.

I'm still sore after fourth course this week (course = injection; 2nd injection after 3-4 days; then "modeling" after another 3 days. "Modeling" is a bizarre term for Dr. breaking up collagen by squeezing the bejesus out of it between his thumbs).

Each time it gets a little better, after this fourth time it looks like my penis is back to normal - very little curvature, length and girth are back. Yay!

What to expect:

- It's very weird at first being injected to force an erection, then having doctor look at it. But I got used to it.

- Strangely, it doesn't feel all that great having multiple injections in your penis, especially when you have an erection. Modeling is even less fun. But the pain isn't a big deal.

- Your penis will look like it's been smashed with a sledgehammer. Don't worry, swelling goes down quickly.

- They tell you to stretch multiple times a day for about six weeks, but my experience is that it doesn't make a lot of difference (although I'm not advising people to stop - it probably does help in ways that aren't obvious). But you'll see what the results of each course are after a few days.

As an indication of how common this Peyronie's is, my urologist said he has about ten Xiaflex doses in his refrigerator there at any one time.

He first examined me, then had me wait a year before starting treatment; apparently it can resolve on its own sometimes, which is better. I had my first three rounds about two months apart, then the last one was maybe after six months (no particular reason I waited that long). At first I had about a 70˚ bend three quarters of the way up, and after the third time it was down to maybe 40˚ but the curve was more gentle. It would have been okay to stop after three courses, but there was still room for improvement. I didn't really notice that it had also become a little narrower and shorter until after this last (4th) course.

The mechanics of how the curve works are hard to understand, but the collagen is in more than one spot, which causes the bend to move even after some of it is broken up. I think the doctor was a little more aggressive this time, finding more spots.

At this point I'm looking forward to two weeks from now when the hematomas are done and I can go back to happy shagging - almost as much as I'm looking forward to this hideous presidential election being over.

One more thing: I don't think I've seen more misinformation going around the internet than about Xiaflex. All that stuff about heat, splints, machines... no. Just find a urologist who knows what he or she is doing and stop reading crap on the internet - other than what I'm writing here. :)

Richard,  congratulation that the injections worked so well for you.  

Think a lot of the xiaflex success depends on the skill of the uro.  You obviously had a good one.

Could you let us know who your uro was or send me a PM to that effect?

I am one week out from my first cycle of Xiaflex and my URO said that I need to wait 2 weeks to model. However, the instructions from the manufacture's website for Xiaflex says 3 days. Did you model after three days each time and did you only do manual stretching? You spoke about squeezing the B----- out of it. Would you explain what that is and did you use a traction device after your cycle? My URO is being very conservative in waiting two weeks and saying manual stretching three times a day for 30 seconds each time. Please advise on the protocol your URO had you doing.

Dr. Robert Dean at Walter Reed National Military Medical Center in Bethesda, MD. If you have military benefits, he is the guy in the DC area.  Wilford Hall Ambulatory Surgical Center in San Antonio is a large medical center that might have somebody trained if you are military.  If you are in the DC area and not military, Johns Hopkins has a few UROs that specialize in peyronies.

James, the improvement is from being bent about 70˚ at a sharp angle 2/3 of the say up before treatments to... well, I can't tell you the angle now, but it's just a very slight bow - the curve is gradual from bottom to top, it doesn't have any effect on sex, and it doesn't look particularly unusual.

If I wanted to go another round I could, but I probably won't bother.

That Xiaflex is expensive! I now have a $40,000 penis. :)

I will second Richard's account. I had a starting curvature of 50 degrees and while, after one cycle, I might have only reduced the curvature by 5 degrees or so, there is a pronounced change in the abruptness of the curve. Prior to cycle one, my curve made a very sharp bend like a door hinge, now the curve is elongated. Just had injection one of cycle two yesterday and keeping my penis wrapped in coflex till next injection on Monday and then keeping it wrapped for another 3-4 days to keep swelling down.
