Alvoip

This will be long and verbose so here goes.

I am in a sort of similar boat with glans irritation, though my skin peels/flakes and is EXTREMELY irritated when touching anything to the point of nearly being incapacitating. I was on Trazadone as a child but there is likely ZERO correlation to my current condition. I also suffer from bad hard flaccid which causes my penis to stick out and rub on underwear or pants, the only time it goes away is laying on my back for a few minutes or rarely sitting in a very well padded chair and slouching forward.

The other co-morbid/odd things I have is multiple VERY inflamed lymph and veins that form a network and have been progressively getting worse for nearly 3 years. My GP doesn't know and muttered something about "congestion" and now I have a follow up uro appointment.

My symptoms of irritated glans/rash:

All symptoms worsened by any light friction, washing, clothing, or gentle masturbation (by far the worst trigger).

Heavily irritated penis head: intolerable when touching any fabrics/friction, low when naked. Raw/burning/sensitive irritation. Washing is painful but lubricated masturbation is okayish during the act (gets visibly reddended during the act though).

Flaking/peeling of entire penis head: First appears noticeably shiny/glazed (almost like saran wrap but thicker) and feels completely smooth  to the touch, eventually gets visible white knicks in dead layer and starts rubbing away over the entire surface and repeats. Very rarely notice a single small patch on the shaft top/right side - only a few times in 2 years.

Swelling of entire glans (or just rim at times): Varies in intensity, uncommonly flushes deeper purple/red and becomes inflamed or tender for weeks.

Rash #1: Centered on top of glans with a branch to the right, encircles urethra.
Varies in visibility at times from bright red/orange/pink to dull copper-red or brown/tan color a few tones darker
than surrounding skin.

Reddness: an overlying red and completely saturated flushing of the entire glans that varies from deep flushing to light pinkish. Also a minor spotty dull copper-red rash of shaft (varies and can be absent/minor or just a dull hue).

On erection: skin is usually very glossy, taught, and veiny (inflammation?) looks like plastic.

The coronal succulus tissue can also be inflamed pink/red after masturbation but like I said I do have lymph/vein swelling so it may just be plain swelling.

Tried the following with no results:

Abstaining from masturbation for 53 days (normal washing) and 30 days (gentle intermittent washing).

Antifungals (Canesten, Lamisil) no difference or worsened from friction/washing of area.

.1% betaderm removed some redness for 1-3 hours and caused it to flush pale white then progresses into discolorations, no notable change in irritation or swelling. Stopped after 2 weeks of a 1 month prescription due to wrinkling and weird texture (skin hardended, cracked, and flaked off - returned to normal irritated appearance after a week or two).

Changing washing liquid, soaps, lubricants, keeping area sweat free, etc. No noticeable change.

The cause of my Peyroine's was likely aggressive and prolonged edging for hours including dry at times even though I am circumcised, I learned recently my maternal grandfather also has Duputryens (haven't seen him in decades). My current best guess is repetitive microtruama, plaque constricting something, underlying lymph irritation, or it is some autoimmune condition brought out by the stress/anxiety of my diagnosis as it presented within a month of my Peyroine's Diagnosis.

~2011: Hard flacdid and perineum pain when sitting more than 45 minutes
~2012: Lymph/veins start becoming prominent and swollen, perineum pain worsens
April 2013: Pain started flaccid (and erect)
May 2, 2013: bending (consulted gp who suspected scar tissue, referred to uro w/ long wait list)
Nov 23, 2013 Diagnosed with "Suspected Peyroine's" (no palpable scar tissue at time of exam but swelling/bending/hour glassing/etc)
Late December 2013: irritation and flaking of glans starts, minor at first and worsens through out the next 8 months.

I did NOT have palpable peyroine's plaque at the time of my exam but was still diagnosed as having "suspected" Peyronies Disease due to hour glassing and 70+ degree bending that varies with erection level, cold, etc.

I waited 8+ months to seek treatment from the glans irritation even though I was in heavy discomfort. my GP initial suspected yeast but antifungals (Lamisil, Canesten) did NOTHING but irritate it more due to increased washing, upon follow up he suspected eczema and prescribed betamethasone valerate .1% which I used for 2 weeks and then stopped due to weird texture changes and lack any discernible benefit... it would flush white for 2-3 hours then look discolored, that's about it.

I see a derm next month on the 23rd (12 month wait list in Canada). If I didn't have such bad flaking/peeling I would suspect genital dysaesthesia too but we'll see. I suffer from blepharitis (diagnosed a decade ago by optho) and have VERY oily skin on my face and scalp. I also suspect sebbohreic dermatits but I think that would have responded well to the steroid. My shins also go through periods of getting really glossy and fine flaking but I don't know if they are related (no irritation just the rare minor itch), I could probably signal aircraft with them.

Follow up questions for you:

1) How is your stress level on a scale of 1/10?

2) Have you had a history of anxious, depressive, obsessive-compulsive, or phobic tendencies BEFORE your peyroine's diagnosis or marriage fell through? did your glans irritation symptoms develop during or around one of these major stressors?

3) how much sleep do you get per night? Do you sleep the whole night through and feel rested or are you tired?

4) do you self exam or touch yourself a lot (feeling for plaques, readjusting, etc)?

5) have you had a biopsy or other tests taken at the derm? or was it just general consultation and exam.

6) how long do you sit per day?

7) what is the longest you have abstained, did it help?

8) since your injury have you been living a sedentary life style or have you resumed sports or other exercise?

9) do you commonly employ aggressive masturbation techniques? lack of lubrication, prolonged duration, heavy grip, etc?

10) How are your washing habbits? do you use soap on the penis, scrub heavily, or use an abrasiveness cloth?

11) no flaking / dry skin?

Nothing I do makes it go away. I know masturbation or prodding make it worse so keep hands off, reduce masturbation, sleep, drink lots of water, and eat well. I'll let you know how it goes in my case.
