Hourglassing, how it originates and manifests?

I remember before Peyronie's, I used to notice hourglassing occasionally when an erection faded. Though I haven't seen too many photos of proper hourglassing.

I don't think I really have it now either in flaccid or erect, though again occasionally when the situation fits, I could appear to have it.

I have at least one (no ultrasound yet) plaque on the left causing the penis to bend left. About 45 degrees. How would the left bend alter if I developed a plaque directly opposite on the right side? I assume I would lose more length and girth? And would it hourglass?

I haven't noticed a topic explaining everything to do with hourglassing, so also please share your knowledge here of how it originates and manifests.

Ah! I just pictured it better now. I used to think I may of occasionally had hourglassing but now I am not as sure. What would happen if this band of fibrosis was incomplete? And just 2 or 3 plaques were present?

I would assume hourglassing first appears after the plaque causing a curve spreads completely around, then I would assume the curve would no longer be present? The penis would appear straighter with the hourglassing?

It can be a full band around causing a full horuglass shape or could be partial causing a partial hour glass on a side. In either event, the band does not form uniformly so there can still be curvature caused in addtion to the hourglass deformity.

Also, I don't believe that "hourglassing" with less than a full or nearly full erection is very meaningful. It could just be the corpus Cavernosum in one part of the penis filling up with (or draining) blood slower than other parts.  I bet the large majority of guys without peyronie's have penises that "hourglass" in some way when an erection is beginning or ending.

Funny that you mention this, but i have been noticing my penis slowly hourglassing sometimes, especially after i urinate and then it goes back to the "post-trauma shape" that it has now. Yet, i do not feel any indentations or fibrosis on that area, or in the penis in general. Do have to give you credit!

Does anyone have any modeling techniques for an hourglass deformity? I've had my second round of xilaflex, and the brochure really doesn't suggest ways to stretch your penis for an hourglass deformity. Thanks in advance!

I believe that curves result from scars that prevent tunica stretching longitudinally (the length of the penis) while hourglassing results from scars that prevent the tunica from stretching circumferentially (around the penis).

Traction and normal stretching don't really help stretch the tunica around its circumference -- the only stretch it longitudinally.  The only thing I'm aware of that could remodel an hourglass is the VED.

I do notice when I use the VED it "pops out" my hourglass while the vacuum is stretching the penis. I haven't used it enough to see if it will help permanently but I have no reason to think it wouldn't.

Around 18 months of VED eliminated the hourglasses.

Hello everyone,

Was wondering how everyone's hourglassing first developed. Did you have curvature for a few years, then notice hourglassing years later?

Or is it always from 2 plaques developed opposite each other (for example)?

I only get hourglassing when I have lower abdominal pain, it can be spermatic cord pain, and also in a more central inguinal region too, though not sure what exact muscle/vessel/nerve is giving the pain.

pfract: I get exactly this too! Hope it is not new plaques developing.

PS:Does anyone know the difference between hourglassing and indentation?

My hourglasses (3) developed slowly at the same time as the curve and bend
I have just one plaque on the left side close to the base and a cord like plaques that makes an upward banana.
So the hourglasses developed not connected to the plaques location/shape
All together made my penis a weird shape

So you would of had 5 plaques before starting VED/Cialis/Pentox. And you are left with 2 plaques, currently?

I know VED rid you of your hourglasses.

So do you think they were plaques at all (the hourglasses), as you still have plaques left. Basically wondering what you think caused the hourglass appearance?

No Pressure, I had, still have, two plaques and 3 hourglasses
The plaques are still there, strong and rigid.
The hourglasses are gone after VED, Pentox and low dose Cialis treatment. I think mainly VED helped.
The hourglasses disappearance made the shape of my penis better. Still deformation, but much less and no buckling during sex.
Why the hourglasses appeared, I have no idea.

What do you think was worse for penile shortening? The plaques or hourglasses?

Am still unsure as to whether hourglasses are mini-plaques round the penis or just a constriction of blood in a ring around the penis?

I first noticed an hourglass shape while getting an erection two months ago, but I have only ever seen it when my penis was semi-erect or flaccid. For me the trigger appeared to be a strenuous leg workout (squats etc.), though I think I am predisposed to it because of pelvic floor issues. I never really saw it again after that when I was semi-erect, but it remained when I was flaccid. It would be very noticeable after I spent some time outside in the cold (Canada in the winter so some days were -20 Celsius). I also had a near constant firm flaccid and occasional turtling of the flaccid penis.

Since I have CPPS and started seeing Pelvic Floor Therapist and she said these were all signs of CPPS even though I had never experienced them in 5-6 years I've had the condition. I guess the strain of my leg workout might have triggered the new symptoms but that's a guess. My understanding is that if you only have hourglassing in the flaccid or semi-erect state this could be a sign of pelvic floor dysfunction and not necessarily Peyronies (though it could be from Peyronies or a mix of the both). An hourglass shape when erect is almost certainly Peyronies.

As recommended by my physio, I've been abstaining from any sex or masturbation and doing deep breathing exercises into the stomach with reverse kegels for about a month. These help relax the pelvic floor muscles. The hourglassing and turtling have basically disappeared and the firm flaccid has improved a lot. I was also on Pentox for a month, which may also have helped.

My point is that if you have hourlgassing in the non-erect state you might want to try treating it as a pelvic floor issue. Even if a semi-erect/flaccid hourglass doesn't bother you, you might want to address it because CPPS can bring up other complications over time.

I think, from my experience, that the main culprit for the shortening are the plaques.
I got back 1.5 cm from the 6.5 I lost, but I attribute that more to the plaques softening (not much softening, but softening) than to the hourglasses that gone.

There are two types of "hourglassing."  One involves lack of blood to the area and one involves plaques.

Back when I was a party guy I used a lot of stimulants the names of which I won't share.  But it was all of them.  LOL.  Anyway, after a couple of days of that I would get severe and extreme "hourglassing" on my FLACCID or PARTIALLY ERECT penis due to lack of blood flow and pelvic floor tightening.  These were due to constricted veins/arteries and the muscle clenching that results from high levels of artificial stimulants in the body.

This "hourglassing" would pass after a few days and the penis' flaccid state would go back to normal.

The second type of hourglassing (true hourglassing IMHO) is when there is internal penile scar tissue that pinches in one side, both sides, or a ring around the penis when it's in a FULLY ERECT state.  I also had this happen when I was having sex and "missed the mark" and the penis bent back at a 90 degree angle suddenly.  The left and right sides began to heal with large "dents" in them where the penis had bent upward.  It was from the internal structures of the penis being torn at these points.

These were 99% healed by VED starting about 2 weeks after the accident.  At first they did not seem like they were going to go away but after a few months they did.  

I believe that dents and hourglassing are caused by scar formation that pulls circumferentially around the penis.  To compare, "bends" or "curves" are caused by longitudinal shortening which pulls the head of the penis toward the base on one side or the other.  Whereas "dents" or "hourglassing" are caused by circular constrictions that pull one side of the penis toward the other side directly across from it and hence restrict the girth in that area.

Bends are treated with traction.  Dents and hourglassing (the "real" kind) are treated with VED.  Though  I think VED can treat bends too (though not as effectively as traction) whereas traction doesn't really address hourglassing so IMHO if you're going to do only one I'd use VED.

I'm not a doctor but the above theories are based on my own extensive experience with damaging (and repairing) my own penis.  YMMV.

Hey James,

Were your hourglasses visible when fully erect, or when semi-erect/flaccid? In my case, the base fills a lot and the second part of the penis doesn't, until is fully erect. Them, all the penis, from base to top is the same width.

crazylord i believe you should try  shearwave elastography to see the exact elasticity of your penile tissue (if a doctor accepts you because i tried and they told me that its not for Peyronies Disease even if i showed them the publications they told its experimental )
