Penoscrotal Plication Surgery

I'm at UTSouthwestern waiting to go back for Penoscrotal Plication surgery, scheduled to start in an hour and a half. A bit nervous, but too busy to be worried. I have a congenital curve to the left of approximately 30 degrees (and a slight upward curve). I'm in my mid-forties, and ready to get it done. I've taken many photos, and I hope to share my experiences here. Wish me luck.

I assume you've come to the decision to opt for surgery after a lot of careful consideration, so I guess you now just have to trust in your doctor's proficiency.

I think a lot of members, myself included would really appreciate you taking the time to drop a few updates here and there. Success stories honestly do work wonders for people who have this condition (or Peyronies) which is often misunderstood with not a lot of apparent solutions.

Once again, all the best for your surgery and I hope you have a speedy recovery mate.

You will do fine, Treee. I am eight months out from my surgery by the same doctor. You are in good hands. I have not regretted once that I went ahead and did it. I wish you the same.

Out of surgery, and in bed at home. Apparently straight as an arrow. I still have the induced erection from the injection. Very painful right now, surprisingly so. But nothing unmanageable. My head is very sensitive, so apparently no nerve issues. Doctor Morey said I did well, wife is very pleased from the surgery pics and my remaining erection. Doc said I gained 2mm, which is good because I need all I can get. Went from 14.0mm to 14.2 mm (5.5 inches to 5.6 inches). Very, very pleased that I did penoscrotal plication. Anything that degloves the penis seems barbaric to me right now.

Thanks for the well wishes, and special thanks to Norm, who had this surgery earlier. I appreciate your advice and PMs.

I had Penoscrotal Plication surgery for a congenital curve this morning with Dr. Allen Morey at UT Southwestern. This is the same surgery that Norm had for his Peyronie's back in December 2013: https://www.peyroniesforum.net/index.php/topic,4840.0.html

I had an approximately 30 degree lateral curve to the left, and a small curve upwards (which I never noticed until Dr. Morey's fellow pointed it out to me at my initial consultation). I've had my curve ever since I can remember, and it is not associated with Pyronie's Disease. Needless to say, it's been an issue with me my whole life (despite a wonderful wife who could care less), and I've had body and confidence issues due to it. I've always wanted to get it fixed and be "normal." (I've sometimes wondered who has it worse: congenital guys who've had to deal with it our whole lives, or Peyronie's guys who got to use it for a while, then had it taken away. I've decided both scenarios suck in their own special way.)

I thought a lot about surgery once I realized that traction, jelqing, etc. usually don't show much result for congenital curves. My penis is fully functional, meaning I get aroused easily and have rock hard erections, can have sex without too much complication (though some positions and acts are not possible), and have children. (I'm in my forties.) Because my penis works, I was very hesitant to have any surgery that was invasive. The Nesbit procedure was off my list before I even started thinking about surgery seriously, due to its extreme nature. Plication surgeries seemed less brutal and less prone to side-effects, but they still de-glove the penis to put in the sutures and they usually mobilize the neurovascular bundle (which terrifies me). I thought about the STAGE procedure, too, which I think is great, but you still have the de-gloving of the penis and moving the nerves. That was just too great of a risk for me.

Then I read about Penoscrotal Plication. PsP is a straightforward plication procedure that uses a 2cm incision on the shaft of the penis, which is then manipulated around the penis to allow access to put in the plication sutures. The sutures are permanent. There is no de-gloving of the penis, and no mobilization of the neurovascular bundle. There's only a small scar to deal with when recuperating, and you can resume sex in one month.

So I finally took the plunge and had surgery this morning. It's still early, but when erect, my penis is straight as an arrow. I've had zero loss of sensitivity that I can tell, and my penis still begins to get hard when I think about sex (though I've not let myself get erect). I had some moderate pain after surgery (~6 on a scale of 0-10), but once I got my hydrocodone prescription a couple of hours later, I've had ZERO pain. I've been doing some light housework this afternoon and evening. (I'm not sure I'll remain pain-free, so I'm enjoying it while I can.) My only concern is that when flaccid, my curve is still there. But as long as it's straight when erect, I don't care. (Can anyone with plication sutures let me know if their curve remains when flaccid?)

Honestly, I'm surprised that Penoscrotal Plication isn't more popular on this forum. It's a great procedure in that it's not terribly invasive, it works as well as other plication procedures, and the recovery time and risk are minimal. I'm only 16 hours post-op, but so far, my surgery doesn't seem like that big of a deal. I'll know if I can reccommend this procedure to others in the coming weeks, bus as of right now, I'm very satisfied. Even my worst-case scenario doesn't look too bad: my penis curve would return, I would have bumps (sutures which could be removed), and I'd out of commission for sex for a month.

Anyway, I'm still trying to process everything that's gone on today, and I imagine that I'll have several ups and downs in my recovery. I may even be ultimately dissatisfied with my results. But right now, I'm feeling positive and hope to share my experiences with the rest of you guys in the coming weeks.

This is great news, Treee! I wish you continued healing. Don't worry about the curve while flaccid, it will straighten with time, and certainly when erect. I, too, was told I could have sex in a month, but I could not. It was closer to two months for me before the pleasure outweighed the discomfort. Don't rush it. I made a couple of unsuccessful attempts before things got normal. However, yours might be a quicker recovery because I suspect you did not get too many sutures. I had eight. I figure you had three or four. He usually puts one suture per 10 degrees of curve. Just remember that even though the outer stitches go away in a week or so, the inside is still healing. It just takes a while. My best advice right now, think about a brick wall. Erections won't be pleasant for a few days!

Just a quick note to update everyone on day two. I woke up with morning wood this AM, and my penis was straight again. I flexed a couple of times, and it was mildly painful -- about a 2 on a scale of 0-10. When I stopped flexing my erection, the pain went away. I'm took a couple of my hydrocodone pills today, but haven't taken one since 1pm (9.5 hrs ago, and I can take the pills every 6 hrs). I've felt almost no pain at all at any time during the day. I even drove the family to a restaurant tonight for dinner, then went shopping. Honestly, it's almost like I didn't have surgery at all.

Tomorrow I will get to take off the wrap and take a shower. I should be able to see the incision, and I might feel for the sutures if I'm feeling brave. I have to keep it wrapped with compression gauze for a week, but after that I don't believe I have to do anything special.

I hope to post more pics tomorrow when I finally get a good look at my Frankenweenie. I'm hoping the scar makes it look tough!

That shower will feel nice. And you will be amazed how small the incision area is. I was unable to keep the compression bandage on after the first change and suffered no ill effects from just slathering some antibiotic cream on it and leaving it unwrapped. Do what works best for you. I was back to work the day after surgery, but I do have an office job. Anything physical would have been tough. Go easy. You'll feel much better in a week.

I finally got to unwrap my penis on day 3 and take a good look at it. I've posted more photos to this topic: https://www.peyroniesforum.net/index.php/topic,5340.0.html

It doesn't look too bad for having been put through the wringer. It's bruised as hell, but the incision doesn't look too bad (the incision is a little less than 2cm, or a bit less than 3/4 of an inch).

I got to take a shower, than had to re-wrap my penis (which I'll have to do for a week). Pain has been minimal. I only feel pain when I have an erection and "flex" my penis via the PC muscle. Erections themselves cause no pain, and I've stopped taking my hydrocodone. The most it's hurt when flexing has been, maybe, a 4 on a scale of 0 to 10.

On day three I got an erection after I re-wrapped myself, and it looked as straight as could be. Today I got up the courage to make it erect while unwrapped. I've posted some photos to the topic linked above. It basically looks like a normal penis that got in a bar fight. The only swelling I've noticed is at the incision site. And it's damn straight.

I've not worked up the courage to feel around for the sutures, but hopefully I'll do that in the coming days. My penis had a slight upward curve (which has gone away), and pre-op Dr. Morey said that he'd put some sutures on the underside to correct that. But externally, I can see no indication that he did that. I should be able to tell when I start feeling around on it. Either way, the upwards curve is gone with no scarring, dimples, or anything else on the underside.

My wife took the unwrapped erect photos today, and she gently helped me get fluffy for the pics. I've had no loss of sensitivity that I can tell. All systems are working fine. I've also noticed no loss in length. I won't get out the ruler until I can get a rock solid erection without pain, but I don't expect anything to have changed.

I know recovery can be a long road, but so far, I'm very, very pleased with my surgery. I still can't figure out why more doctors aren't doing Penoscrotal Plication for both congenital curves and Peyronie's. I'm very thankful to Dr. Morey at UT Southwestern for his skill and conservative approach in developing this procedure.

For you guys who had this surgery: Did you regain some lost dimensions in terms of girth/length? I've already lost more than an inch of length + another inch of girth in about 4-5 months. Can this help restore it?
