Don't know if this has been posted here before, forgive me if it has, but I found another long term (2 year) case study using L-Arginine, Pentoxifylline, and PDE5 Inhibitors, this time in a younger patient.  It's encouraging at the very least to keep trying this therapy.

Good to hear that! I've noticed things feeling a little better erection-wise when I combine the L-Arginine with Pycnogenol, I can clearly see that the tissue is more relaxed and fills more readily when I have an erection after about a half hour or so of taking it. (I should mention this is also after taking my daily tadalafil) It's weird, but I can feel the blood kind of passing over the plaque in a tingling type of sensation, so I think it's a good thing. It's doesn't last all day though, only for a few hours until I take it again. I still have all the problems erection wise, though i feel this helps and in the long term I'm hoping it will bring some changes as cells divide, go through apoptosis and whatnot (hopefully myofibroblasts) . I've been taking 3x1000 mg L-Arginine twice a day with 1x60 mg Pycnogenol with it also twice a day on an empty stomach, I've read in another study somewhere that Pycnogenol really helps with more Nitric Oxide availability. I personally have noticed it in my experience anyway, there is definitely a change for me, though it's not permanent. I'm debating on getting some L-Citrulline somehow to swap out the the L-Arginine, for further NO and to see if that works, though I haven't found it anywhere.

Citrulline Malate is what you're looking for. You can read up on the differences between the two on the net. If you do buy it, I use it personally, it helps me greatly with my gym work, buy it in powder form. It's far cheaper and easily dissolvable in water.

It was a one person study of a 20 year old male over the course of about 2 years.
Here is the summary at the beginning of it:

Objectives: To evaluate the effects of upregulators of nitric oxide in one patient with
Peyronie's disease, after non significant improvement following intracavernosal verapamil.

Methods: A 20-years-old Caucasian male presented with penile induration in flaccid
state, persistent during erection and associated with mild pain at third middle of penis.
After treatment with intracavernosal verapamil for 4 months with relief of penile discomfort, followed
by counseling on the use of penile extender for at least 6 hours per day, he was prescribed
pentoxifylline associated with tadalafil plus levo-arginine, propionil-carnitine and Vitamin B3.
Results: After almost 2 years, the septal thickness was reduced at ultrasound evaluation after
this "triple oxygenant therapy".

Conclusion: NO-iNOS biology in Peyronie patients is the very protagonist in modulating penile
fibrosis through up-regulation of NO-cGMP pathway

Thanks for the clarification.
The problem is that a study on one person is really not giving to much.
It is one person that have improvement and with Peyronies what working for one is not assurance it will work for others.
I am stating that from reading all the posts posted on this forum from the very start.

I agree with Jonbinspain. Citrulline Malate is better than Arginine.

Citrulline Malate

This amino acid is so closely related to arginine that in the body, it is actually converted into arginine. However, citrulline may boost Nitric Oxide (NO) levels even better than its cousin because cells in the digestive tract are always starving for arginine, and they tend to grab it before it can get into the bloodstream and be converted into NO. The intestines are not interested in citrulline in the slightest, however, which means that it passes straight through them and into the bloodstream. One study, published in 2008 in the British Journal of Clinical Pharmacology, found that when subjects took equal amounts of citrulline and arginine, the citrulline increased levels of arginine and NO in the blood more than taking arginine did. Suggested use: 3 to 5 grams 30 to 60 minutes before sexual activity and workouts.
