Have somebody on the forum malleable semi rigid implant?

Why most of the people not choosing them?
I read some very positive posts on an other forum regarding the malleable implant.

Well they are much cheaper than the lgx, but what I read is that you allways have a semirigid erection. it does help, but when you dont want it its still there. It certainly is better than nothing but you certainly cannot control it.
I think jackp can tell you more.

newguy

Have any members here had a semi rigid implant? If they are cheaper, I can see why some of those requiring implants would go for that option. Always being semi erect would no doubt be unusual at first, but I'm sure it's something a person could adjust to.

Luciano

Well at the mayo clinic homepage they have a comparison table of all types of implants. with pro and cons.
I have no idea how reliable they are.

http://www.mayoclinic.com/health/penile-implants/MY00358/DSECTION=what-you-can-expect

QuoteSemirigid

Semirigid rods are always firm. The penis may be bent away from the body to have sex and toward the body to conceal the device. Although less commonly used than the inflatable type, semirigid implants are less complicated, easier to place and have less risk of failure.

pros:
Requires the least extensive surgery of all implant types
Has fewer parts than any other implant, so less of a chance of malfunction

cons:
Results in a penis that is always slightly rigid
Is more difficult to conceal under clothing than are other devices
Puts constant pressure on the inside of the penis, which can cause injury in some men

its the last of the cons that would worry me.

I read some posts on an other forum dealing with ED.

Some people, including people from the US that has done the semi rigid implant are very happy and they say is full erect, not half erect.
In Europe have many people doing the semi rigid implant, maybe because the doctors don't need to be so skilled like with the inflatable.
Sorry I am not quoting what they have written, I don't want to do without permission. I will try to ask permission and quote here.

I try to figure out the subject to understand why still many people are doing the semi rigid implant.
I just read by the Internet that in the US 45% are doing the 3 pcs implant, 20% the 2 pcs and 35% the semi rigid.
If it is so bad, why still 35% are using it? Just wandering.

Regarding reliability they are the most reliable as they don't have reservoir, pump and pipes. Just two semi rigid roads.

Hey guys:

There are to my knowledge three types of implants. 1. The semi rigid, 2. the self contained one that can be inflated by pumping the self contained fluid, and 3, the three piece unit.

Number one stays in the same configuration all the time and can be cumbersome to deal with concealing it, etc. Number two can and will cause further trauma if not carefully inflated and deflated after pumping up. Number three has the best opportunity of correcting ones ED, but requires more surgery and than the other two types. Cost for the three piece implant is much higher too.

The above is just my 2 cents based on reading about the comparison of all three on a medical website.

I get permission to quote so here I am:
QuoteI have the "semi-rigid" malleable implant...Having a hard piece of wood 24/7 has it's positives and it's negatives.
You are ready all the time! BIG plus.
Bending it out of the way can be a hassle on occasion.
So far my wife...likes the new toy very much.

Some are less happy:
The implant generally at 70% erect state... &
Most times the love making is good but I am very limited when attempting some...

Like everything with Peyronies, for some is excellent, for some less.

I just had my semi rigid implant(AMS Spectra™ Concealable Penile Prosthesis) on 4/17/2012 at Stanford university. I am 35yrs old(African American) with Type 2 Diabetics still working on my control but i had Peyronie Disease with a little damage tissue. It has been great so far and been able to use it couples a time a week. Only down fall is that i am unable to bend the implant down all the way? So its press up against my belly button. Does anyone have any tips on getting the implant bend down all the way.

Thank you for the post (personally) and welcome to the forum.
Sorry I can't answer to your question as I am just planning to have an implant.

It may be that I will not be able financially to have an other implant that the semi rigid one. I would like to ask you to answer a few questions:
* You have lost length and girth to the Peyronies? If yes, the implant give you back someting from the lost size?
* How is the shape of the penis after the implant compared to before?
* Are you happy with the rigidity?
* Is your partner happy with the results?

James, I'm on my 10th day pos-op and i can say to you that rigidity is excellent.
I still have natural erections associated (of course not in the same way) because my doctor put the implant on the periphery of corpora cavernosa. I had moderate pain in the first week, used 1 or 2 times opioids just to sleep. He rectified and stretched my penis because i had lost 2-3cm before. Now i'm circumcised, my glans is keratinized and i think that i will lost a little the sensibility, but i don't worry about this. I just have to not bend it down for one month.
My only question to whom already have the implant: Someday will I forget and stop to feel that i have an implant? Will i be able to bend it down without pain and hide it under my clothes?

james1947

tommarkey

Thanks for posting your experience.
Regarding your question, not from my experience, but from what I have read on this forum and other, you will be able to bend it down without pain. You will also stop thinking about the implant as a burden, you will think much more like a superman. 8)
An you will feel same when you are going to the man toilet 8)
Please let us know your progress.
I was on the way to make the implant, but had some improvements with the oral treatment & VED and each time I am deciding to go ahead with implant, some higher priority issue come on :( (my daughter 3 year old, myself 65 ;D)

Wish you fast recovery and very pleasant usage.

Hi there. I had several issues but have been resolved through surgery. I'm in my 4 month after surgery, and couldn't be happier. I feel like I have been given a new lease on life. The sex is better than ever, and my girlfriend agrees. I even feel more confident than before. My problems have started with ED, then with shrinkage, and later in discovering that i had two huge plaques. I've found a great andrologist/urologist her in Rome Italy, and had surgery. They removed the plaques, inserited semi-rigid implants, complete circumcision, and then also cut the suspensory ligament, to regain some of the shrinkage.

The first month took time to getting use to, and lately i find it more confortabile if i keep it to either side. It hides better if keeping it up towards the belly button, but is uncomfortable with the elastic wasteband in the way. I too can't bend it in the down position, and was hard to go back to the gym. I would try to hide it, but now I don't give a damn. Its 3 months since I'm back in the gym, so I think they think its normal for me.. Then again who goes to the gym to check out other peoples junk. I also find that wearing a size smaller of slip helps conseal better.

Just an off topic for Peyronies... I also have type 2 diabetes, and as suggested by my doctor, that diabetes could also be linked to Peyronies. It would be interesting to know how many members here have diabetes type 2.

James, thanks for your response. I'm observing some of these facts. I will tell about my progress periodically :)
Well, a little more from my history: I'm on my 22th day post op now. What i can say is: Why I didn't did this before? I'm 23 years old and had peyronies for eight years. After years trying a solution i think I'm happy again. (I used  propoleum, VED, penile extender, cialis, pentox... And I waited for xiaflex until the end of my pacience). My libido is better (yes, i know that the implant does not give it better, but it worked to me!). My night dreams came back again. I didn't have sex yet. Anyway, I don't have a girlfriend and i just can do after six weeks.
I don't accustomed myself yet, but i think that is just time. I'm just with some difficulties to wear a briefs. Elastic boxers are really good to conceal the penis against the body, especially smaller sizes, but they tight a little and I can't use cause i have pain... maybe just now in the begin. I don't know how to say it in english, but here in Brazil we have a kind of briefs called "samba-canção" It looks like elastic boxers, but is more confortable (like this https://metrossexuais.com.br/lojavirtual/image/cache/data/Jog/PDF/Samba Canção Sortida 906 – Jog-500x500.jpg). --> Copy and paste all the link
This is the most confortable briefs, but just to wear at home. They don't conceal anything.
Friends, what kind of briefs do you use at your routines? and how do you position the implant for better confort and concealment? My doctor said to me to wear a slip and bend the penis against inguinal region. However the slip don't conceal like boxer and I have some pain after some time with the penis in this position.
I tried to bend it down now, but i can't too.. My doctor said that this is just for a time :)

AFMSR and jean claude: I was talking with a patient of my doctor that has did the surgery 1 year ago. He said to me that you have to move your implant to get it malleable. After the surgery, he left his penis down and to left with some (not to much) force in the briefs. Three months after, the implant naturally bend down and he could dress the underwear without any neurosis

tommarkey

25th day: Friends, sorry for the early uptade, but I'm having a lot of progress in few days.
I'm manipulating a lot my implant. I must say to you that you don't have to fear this, just do with careful. I can bend it down now perfectly, and if I want I can stand it down too, but with the force on the briefs, not naturally yet. Bend it down is not the most confortable and concealing way to me. Some days ago I was bending it to the navel. Concealing is good, but is not a confortable way to use jeans or other tight clothes, especially because the pain that button cause at the glans. Now i'm bending it in the inguinal region. Concealing is good with jeans and does not looks like erect, just that i have a bigger penis. I had some pain with jeans and elastic boxers, but it is decreasing. Now I use cotton boxers with wide pants for confort at work (just for a time) and sometimes the jeans with a elastic boxer.
I'm having natural tumescence at night, like a sponge. Well, I'm just very happy

My implant now stays down naturally, without manipulation and I'm wearing my slip like before the surgery. My penis goes naturally to the left in it. It stays always in the same position if I don't manipulate. Don't have any pain anymore. I feel like I have a flacid penis (bigger than before)
Well, it's important to say that I can't have sex standing up, just in the bed, cause my implant falls in this position. I can stand with the penis in my hand if I want but I don't do this. I was warned about this fact. Really not an important thing to me.
I can say to you that if i need a new implant someday (ten, twenty or thirty years in the future) I will choose the malleable. It's simple, i don't worry about mechanical failures (a more important problem through the years with the inflatable one, and I'm just 23 yo). I have some gains in girth and lenght with my natural erection, the rigidity is very satisfactory. I can hide it perfectly at my routine and most of the time. I went to the beach last week. It's a little visible but few people pay attention and most of them just think that i have a big penis (not a problem to me). You can have a happy life if you don't give a damn about this.
Another important thing that i have to say for whom choose the implant in the future: You must be patient at the first month. After 35-40 days, all come back to normality. You must control your psychological status. It was difficult to me in the begin but now all is well again.

james1947

tommarkey

Thank you for the answer :). It is enforcing my decision to go for it.
I have one more question: did the doctor cut the ligament that holds the penis up to make it longer? If yes, this is the reason that the implant falls while standing. If not, I don't understand the reason. Can you elaborate in the subject?

No, he did not cut.  In my diffuse case, he did a genital reconstruction. He lenghtened the penis into the limit of neurovascular structures, reconstructed the tunica, circumcision and implant... I gained again what i lost with Peyronies Disease. It's necessary a malleable implant in the first six months to prevent constriction, and if the patient wants, he can change to an inflatable one after this period (I will not do this)
The implant falls because of gravity. It is very malleable after some time. I have a good and firm residual erection, but not sufficient to bend it up. I can explain better to you: The implant just stays firm at the limit of 90 degrees from body's plan. You can't have sex with 180 degrees (standing up), unless you hold your penis with your hand.

Well, what do you want to know?
After almost three years, I remembered the existence of the forum some days ago.
What i can say is that my satisfaction is complete.
I feel like i never did anything before. I can have erections like before, but now without curvature and full.
All the women i had sex since then, a lot, don't noticed the implant.
The implant has axial rigidity and radial rigidity in the begin, after surgery. With time, radial rigidity dissapears and only axial's remains, the necessary for intercourse. So, i don't feel a rigid penis, unless i have an erection. I have 100% of tumescence around the implant. My doctor said that this is because he putted the implant above the center of corpora cavernosa, and not at the center.
I can go to the swin and beach normally. Yes, people notice a huge penis at my bath clothes! But it does not matter to me anymore, after so many time!
But i did not had this information when i did the surgery.
It taked a lot of time to understand these things that i'm saying to you.
It taked some months to adjust myself at the situation after the surgery.
I know what you feel with this disease, friends... So i'm here and strong recommend this surgery to you!

An important thing to say: I can have intercourse as long i want, but intercourse is not sex! It is just the mechanical part of it. As any men, i need desire and excitation to have an erection! Without this, i don't feel anything. With this, i have normal orgasm and ejaculation. So, the brain remains the most important thing for the sex!

Sorry for my english. Today i'm in the cell phone and don't have the translate help. Hope you understand

Brother, I m not able to send you a personal message it says your inbox is full.  i have 2 more queries
1. Were your erectile tissues removed for making space for the implant
2. Does implant limit your physical activities in any way
3. How is intercourse with a malleable prosthesis?
4. Is the urinating same as before?

Shiv

I just got the Coloplast Genesis malleable penis implanted. Some comments:

1) 13 mm cylinders give fabulous girth, but are too "intrusive". It's like steel pipes pressing against the side of the penis.

2) A 11 mm or even 9 mm cylinder would be more comfortable. I think the girth would be very good even with the 11 mm cylinder, perhaps a bit on the slimmer side for the 9 mm, though tolerable. Please note, my flaccid penis was not too large, maybe 2.5 inches, so the 13 mm rods are perhaps for larger penises.

3) Size in flaccid state is pretty good, girth is really good - there is a gap between the corpus cavernosa cavities, so the total girth is rather bigger than the combined girth of the cylinders.

4) I'd rather "undersize" the penile device - this needs to be more invisible. Also, I do feel the rods in the glans, I'd recommend that the surgeon trim it and leave at least 3 mm gap between the surface skin of the glans and the cylinders. It may make the glans act in a "floppy" way, but is perhaps better protection against erosion due to the forward movement of cylinders.

5) The Genesis is VERY stiff. I wish they made is more "malleable". Positioning it is a challenge, you need loads of time to make it more malleable (perhaps). A totally soft silicone implant may be less rigid but I think a lot more flexible. This is highly stiff, not a good design.

Not too happy with this one, I think an undersized version with 9 mm or 11 mm rods would be better, maybe the rods need more trimming near the glans.  (You have to look at the penis size in flaccid state to judge....) Also, this needs to be softer and more adapting.

For pain killers, I needed hydrocone-ibuprofen 7.5 MG 200 MG. I think you may need up to 2 such pills every 4 hours, occasionally with a high-strength Tylenol.

This is a controlled pain killer, your doctor needs to provide a handwritten, ink-signed prescription (it can't be called or faxed in Illinois, that's a real pain and I suffered a lot due to that!).  I needed at least 2 weeks worth of pills, and I'd estimate 12 pills a day. **Remember, ibuprofen and Tylenol in high doses can harm the liver, so make sure to research this closely and get your doctor's approval (it anyway is a prescription medicine).  Also, listen to your body, if you feel nauseous, then try other pain meds.

Another way to reduce discomfiture is positioning. Place a small pillow in between  your knees, sleeping on the side, etc. You may need to remove undergarments for comfort.

During the surgery my surgeon asked for Marcaine (hope I'm spelling it right) as a local anesthetic in addition to general anesthesia (I believe....). It worked like a charm, immediately post surgery I was doing amazing. It took at least around 12 hours before things hit me a bit hard.

The right meds and position in bed will really help. Also, buy some cream for the scrotum with licodaine or pain killer, and keep the area clean with anti-bacterial lotion or cleansing pads. Taking antibiotics daily is also a must...

You also will need someone to nurse you to health and support you.  Take at least the first week off from work....

james1947

Thanks James for your welcome.

I'm doing better about 4 weeks post surgery. Still a bit of pain once-in-a-while....both the scrotum and the penis can sometimes hurt, but not as bad as before, certainly some healing has happened (the pain may be up to 5 or 6 out of ten at times, and you need to take pain meds then and shift your position to a more laying down stance, even when seated...).

The penile implant (Genesis) feels less stiff, and the penis now "comes down" more. It means that it's not sticking almost parallel to the floor, but slants downwards when you stand, though it's still noticeable. You wouldn't be able to wear shorts (or trunks!) with this one. The front part of the enlarged penis won't bend inwards....which is a pity.

I think that the 4-6 week healing period for such surgeries are perhaps the standard time. The penile device still feels a bit on the larger side, I'd like to stress that smaller rods (11mm or 9mm) can be considered, the girth would be fine for at least the first.

I need to still keep the hydrocone-Ibuprofen medication with Tylenol for additional shoring up. But I need to use it a lot less, may be 4 - 6 pills a day.  It's still not too comfortable to walk for long or sit in some positions for long.

The tendency to be nauseous and throw up sometimes comes in a pretty uncomfortable way. If I don't have any food for the day, I still sometimes need to throw up with barely anything coming out, just a bit of stomach fluid. That's probably the result of pain meds or antibiotics, and it's uncomfortable (this may happen even if you reduce the dosage greatly). It's not often, but such states may occur....

As a tip, hydrocone-based pills may cause indigestion, so ask your doctor on that one for supporting digestion-related pills. This may be the cause of some of the nauseous feeling that may come in from time to time, again it's not too often.

There is some auto-tumescence - the penis does naturally engorge with blood, the glans can also grow. So I think that things like Viagra, Cialis and Stendra may work here, certainly Trimix ought to (though it's highly acidic). This tendency to get tumescent conditions increases if you stand or walk, use male hormones (androgen/testosterone), etc.  Laying down reduces this condition, so gravity has a lot to do with it.... Initially, this auto-tumescent state where erectile tissues swell can be uncomfortable, over time it may improve as pain decreases.

The "ever erect" state is less noticeable, in the first few days it's much more prominent. You don't find it comfortable and the use of pain killers and wearing loose clothing helps. But over time, your penis starts feeling more natural - in my case, the rods seem a bit big (I chose 13mm, great girth but I'd rather go with a 11mm or 9mm - trust me here, you want to undersize the device more even if it means poorer aesthetics).

For most part, I don't feel that I'm in a state of erection, which is pretty stiff but also - as noted - it's now willing to slant downwards. The penis is not really falling all that naturally (sticks out too much), so that's something you have to get used to. The device is less malleable than you like, you can bend it only in limited ways, and it's not got enough soft and pliant "play room" to it. But if you can forget it exists then it won't matter too much.

I do want to add, you need a lot better healing then where I am to be comfortable with this decision. But in part, it's "psychology" that holds up. For the first 2 weeks, I'd advice that you always have a family member help you with basic things - food, etc.

This is not really something I'm truly comfortable with, it's a handicap in some ways. Maybe more time...or perhaps a tinier and softer device would work out better. I still think that my cylinders are a bit too big for my penis.

Thanks again for the details Shiv
I had a prostate surgery and a bladder surgery15 months ago, the only pain killer the doctor (Professor, Head of Urology department) was ready to give me was Paracetamol. He told me I can take up 4 gram per day and have no side effects or addiction problems.
Wish you fast recovery and happy usage :)
Just one more question, how is the rigidity and shape compared to a natural erection?
Please continue to update us for time to time.

James, the rigidity is good, almost as if there was a semi-metal pipe below the skin. It's the flexibility that's more the issue.

Shape is excellent, though when it swells up near the glans (like when you sit for too long) then it get's malformed near the top. You can use gauze and wrap the penis to reduce such swelling.

Many times the swelling goes down....not much swelling then...and then the penis looks really good. Fine shape, girth, support, etc. With the swelling the top seems a bit enlarged just below the glans.

You notice that the rods are in the glans, and I'd recommend to use slimmer rods (cylinders) and leave a few mm gap between the surface of the glans and the rods - though it may compromise aesthetics and result in a "floppy" penis top.

Thanks Shiv

What about the length as MoatazKad has asked?
Did the surgeon cut the suspension ligaments to give you more length?

No, that's done for penile lengthening surgeries, which have their own risks.

With the AMS LGX device, you can increase length over time, like 6 months. The Coloplast Titan is more girth oriented, it's also stiffer while AMS is said to be more natural.

When you go for a malleable, your flaccid penis length = erect-state length. Maybe in cases where you have tumescence, there could be some more size (glans can grow, etc.) due to blood engorging in penile tissues.

So if you want more growth you could try Cialis or Viagra or Stendra, but all of these may have side effects on vision. Not worth it, I say...

Don't try to inject Trimix, etc. for a couple of reasons:

1) The corpus cavernosa is fitted with cylinders, you don't want a super-thin injection needle scraping against it!

2) There is a greater chance of infections. For the first 3 months, you should not do surgeries, biopsies, etc. near the penile areas.

In fact, even dental surgeries such as root canal, getting a bridge, etc. should be done after 3 months, and you should check with your doctor if special antibiotics are needed for dental treatments. In that case, your dentist may make you take several antibiotic pills an hour before oral surgeries.

I'd also recommend that (with medical prescription) a light but constant antibiotics treatment be maintained for the first three months. This is even if the penile implants are coated with antiseptics - as Coloplast devices tend to be, which should help for the first month or so. Any antibiotics treatment must be measured to limit side effects.

what does cutting the suspension ligaments achieve? great flaccid length?

Shiv

Yes,,.but also more erect-state length. It takes a while to be achieved and has risks. With the malleable the flaccid-state length goes roughly to the erect-state penile length....so you always have a bigger penis.

Wow, this is very informative, tommarkey! Thank you for updating us!

What is your surgeon's name?
Something that concerns me about semi-rigid implants is the chance of, with enough time, eroding the glans. Did your doctor talk to you about this?
Since you are young (like me) this is a real concern.

Um abraço!
