Hourglass hinge horror

I am 59 YO, noticed weakness (hinging) at the base of penis mid 2019 and indentations early 2020, progressed to hourglass with deeper indent on right than left, and more severe hinging w/ 30 deg bend to right and 2 cm x 1 cm hard plaque in center dorsal area at base of penis. I have no ED, though I believe slightly lower rigidity at my age contributed to the bend trauma that probably caused the plaque. I have had 2 rounds of Xiaflex and this has decreased indents, and the hinge is a bit better, but penis is still too unstable for sex. First round of injections went ok, pain of injection without numbing was bad but very brief. The bruising was worse on the second injection 2 days after the first, and was significantly worse on the second round - especially the 4th injection. But nowhere near as bad as some have reported. Will get the third round in 5 more weeks.

I greatly appreciate the time and advice some of you have put into helping the rest of us. I'll keep you posted.

It's great it's starting to help.  Many docs believe adding traction between rounds increases the improvement.

Welcome Shane,  You are among those that completely understand what you are going through.

This will sound like "I have one so you should get one" BUT this is my take.  If you are 60 with less than firm functional erections the odds that will improve when you are 65 is slim.  An implant would correct your bend, hour glass, and totally make a lack of rigidity a thing of the past.  I suspect that if you continue with injections, nurse along with instability, that you will cause yourself pain, time, money, and possible reinjury when one outpatient surgery could fix it all and DONE.  NOW.  that does not mean rush into an implant.  Read some implant journals, ask questions, pick a high volume surgeon by asking the right questions. I farted around for over a decade and after I got an implant I felt like a fool for not doing it sooner.  To the man, I think every guy here with an implant wish they had done it sooner.

While your researching all your options I'd also consider low dose nightly generic cialis (2.5-5mg) as it can help w stronger erections and remodeling plaque to healthy tissue.

Many thanks to both of you for your quick responses and advice. I will research those topics as you suggested. I had thought about low dose cialis and will order some up. Do you think traction works for hour glass? The mild bend I have is not a problem. My urologist has experience and I think he has done a good job with the first two rounds of Xiaflex. I have a rather hard plaque that was at least 2.5 cm x 1.5 cm and he is injecting in multiple locations each time. the plaque is now perhaps 75% of the original size.  But I think I may need to see a more experienced specialist

Although a very small study, there is evidence that traction can increase girth and reduce hourglass deformity.  you could also consider a 3 cylinder Agusta VED.

I have a dorsal plaque at the base of my penis, causing hourglass indentation and hinging (very little stability when I am hard). It was about 3 cm long, 2 cm wide, hard but not calcified yet. I have had 3 series of Xiaflex, just finished the third. After two series (4 injections on the right side of the plaque) the plague reduced to about 1.5 cm x 1.5 cm and the indentations are smaller. The bruising and pain gets worse with each series, but it is tolerable with ibuprofen and ice the first 1-2 days and heat a few times a day for 1-2 weeks. He injected on the left side this time and I will wait to see how things look in a few weeks to see if I will go withe the fourth series (and last one that my insurance will cover).

Hope things are going well for you Shane (sounds like we have a rather similar deformity- not fun!) Had any luck in reducing it further by any measures?

Hawk- don't suppose you recall the name or authors of that study showing that traction can exacerbate girth loss/hourglassing? Was considering starting traction but would like to read about possible negatives before I do so. Feel free to message me the link to that study if easier.

Most of us have some degree of indent/hourglassing. I have xiaflex scheduled in 4 weeks from now. I will be getting it and doing restore x daily traction over my 4 rounds. I will report if my indentation/hourglassing will reduce. My urologist said, xiaflex should help but in case it it not sufficient, he can do extra tunical grafting to add stability and prevent hinging and hourglassing.

I owe you an apology.  In my hasty typing, I was unclear and gave the opposite impression that I intended to give.  The study indicated traction increased girth and decreased hourglass deformity.
