I've had a severe PMO habit for many years.  39 years old.  Finally started having PIED just about 2.5 years ago, though I didn't recognize it as such at the time, just thought it was normal ED. I also started having some pain in my penis occasionally while masturbating and finally some flaccid pain beginning about 1.5 years ago.  But it wasn't until about 6 weeks ago I started to notice a couple tiny hard flat spots up near my glans on the underside by my urethra.  Finally, just about two weeks ago the combined weight of my worsening PIED, the pain and the hard spots prompted me to start doing some serious googling of my symptoms. Discovering the existence of Pyronies, I became extremely concerned and - upon further inspection of my dick - discovered to my horror some (minor) denting and hourglassing on the left side of my penis (the only side that's ever hurt).  

That did it. I was convinced of my self-diagnosis. I was also convinced - and remain convinced - that relentless PMO after the onset of PEID (aka flogging a wet noodle) caused my case.  

Anyhow, my Peyronies is complicated by PIED.  It's hard to distinguish which symptoms are coming from which problem, and it's easy to see how each could be worsening the other, as PIED reduces blood flow and oxygenation in the penis, which (from what I understand) worsens Peyronies, and Peyronies can of course cause or worsen ED.

Anyhow, I did a few days of obsessive, horrified research on this forum and the broader net and rushed off to an urgent care doctor to request Pentox and Cialis.  I also bought a SOMAerectStf and am trying to figure out how to use that (not succeeding too well, feeling some discomfort, excessive swelling at the base, excess skin getting sucked in, not getting anywhere near full, etc. Worried I'm making things worse with it, honestly, but all the testimonials on this forum are so positive I feel I need to press on.  So I'm hoping to figure out what I'm doing wrong.)

So, some combination of the Pentox and Cialis I'm on (along with a ton of daily ibuprofen, drastically improved diet, no alcohol, acetyl l carnitine, fish oil, CoQ10) and total abstinence from PMO have stopped 85% of the flaccid pain I was experiencing, and things are generally hanging more loose and full down there.  

However, taking the cialis before bed, I've started having some nighttime partial erections, which I frequently wake up to in time to check.  And the hourglassing seems to be worsening, which is obviously really scary.

My fear is that things are going to only get worse from here.  I'm pretty obsessive and panicky about it, and my moods are careening around in conjunction with every thought, sensation and glance down at my shaft.  

I'm especially worried because I have two problems I want to solve at once: Peyronies and PIED. And it seems the solution to PIED - abstinence from PMO, which was/is my only route to erections - might be at odds with the optimal treatment regimen for Peyronies, which would probably involve more frequent erections to better fill, stretch, circulate blood to and oxygenate the penis.

Does anyone have any advice for me on any element of all this?  I would love to hear it.  Also, if any of you have had very similar experiences with this disease, I would appreciate hearing about your case and how it might compare with mine and prepare me for what to expect going forward.  I have urologist appointment tuesday.  So, hopefully the doc will be helpful too.
Age 39, not sure how long I've had Peyronies. Still hoping maybe I don't have it, but denial isn't just a river in Egypt.

Why don't you try traction or vacuum? I am guessing that vacuum would get some blood moving, but you might have to ask a doctor.

When does the Erectile Dysfunction strike you?

Having Erectile Dysfunction while at home alone is different than with a partner. I used to worry a lot about Erectile Dysfunction but got over it as my anxiety went away and I stopped masturbating so often. Traction and abstinence did wonders for me, but I don't recommend prolonged abstinence either.

Avoid masturbation binges and slowly let your brain recover.
