Has anyone of you heard about the modified sliding technique? I know it's been done by doctor Kuehhas and dr Egydio, but there are more doctors that use he technique. Supposedly it could be combined with an implant and give you extra size (back). About 2cm of length could be gained and I'd be very happy with that personally. My main issues are hourglassing around the glans, venous leak and I lost some length and girth while my penis also got more deformed over time. The thing I'm afraid of is more scar tissue from an implant and being stuck with a smaller size. I have about a 15cm erection left where it used to be 17/18cm long. After having struggled for a long time I might want to go for an implant because I'm tired of living with this, but if possible I would also like to regain the size I lost. I also worry a bit about the possible extra scar tissue. My penis for some reason is still inflammed so maybe an implant would make it worse? I'm not sure and neither are urologists it seems because I never got a clear answers what is wrong except that there is scar tissue, I probably have a venous leak and I do have pelvic floor tension. What could I expect from surgery? Will it be worth it for me or should I keep waiting for something that might never happen? I hope you guys can help me out a little. Thanks in advance!

I'd get a consult w a high volume implant specialist.  I think the best way to maximize your length is to do VED and/or traction for 8+ weeks before the implant and then go to a doc who is sure to maximize your length.  I've heard nightmares about the sliding technique, so I'd be Leary if that.

Can you tell please, what exactly did you heard about sliding technique? Did you spoke personally with someone who had underwent this procedure? Or some doctor told you some information that has not been publicated officially?

I believe I read a bad outcome in this forum.  That, and they don't do it in the US.  Also, I've asked you specialists who told me any penis lengthening surgeries are risky at best.

I would also like to know more about these horror stories. What doctor would you guys recommend when it comes to implants? I only care about the end result, not necessarily about the cost or location.

The only surgery I am aware of for lengthening that might be considered by a good surgeon in the U.S. (don't know about outside U.S.) is if someone has a micro penis condition (generally under 3 inches in an erect state). It doesn't actually lengthen the penis but a ligament is cut between the pelvic bone and the penis allowing more of the penis to be outside the body giving the impression it is longer. One can expect no more than an inch in longer appearance. There are some wellness/outpatient facilities advocating injecting fat cells to create more girth but the results have been spotty at best and in my opinion should be in the same class as PRP injections.  If there really was any surgery that could lengthen the penis the person would be a billionaire and we would all know about it.

The MOST technique coupled with implant seems like the most plausible of what I have seen but is very invasive and will carry some significant risks of potential nerve damage.  Please note I am no expert and I am sure others know more than me.... so this is based on my research and conversations with several leading surgeons.

I hear lot of good stories about dr Eid. What can I expect if I go trough with an implant? Will I lose length and girth and is it even possible if you have hourglassing and scar tissue around the glans?

Popopo- I recently had implant surgery with Dr Eid (March 28) and there may be some info to help your decision on my implant journal.  I will also try and answer any specific questions you might have.  I had (have) a pretty severe case of Peyronies Disease with significant hourglassing. You can see my pre-op picture on my journal and you will see it visually. A crucial part of the process will be the doppler sonogram that Dr Eid will do the day before surgery if you are coming from out of town.  He will be able to tell you what length and girth you should expect post implant.  Generally (note it will differ by patient and severity of Peyronies Disease/plaques) one will retain their stretched flaccid penile length post implant as long as one follows the post op cycling routine prescribed by Dr Eid. Dr Eid also has some unique approaches to the surgery (e.g. positioning on the operating table, using spinal vs general anesthesia) to help maximize length.  He will be as aggressive with implant size as he can.  I have regained quite a bit of girth post implant.... inflating the cylinders essentially expands the tissue where I had the hourglassing. I was told that I could expect a length of 13 - 13.5 cm post implant and am currently between 13.5-14cm.  My curvature has gone from 90 degrees upward to 20-25 degrees upward. This is 26 days post surgery.  I expect that I can get to 14 cm and approx 10 degree curvature within 3 months.  Let me know what questions you have.

Hey Curved, thanks for your reply. Coincidally I just checked out your journal today and I must say the outcome of your surgery looks very good. A more natural shape and no sign of scarring from the surgery. Did your sensitivity change for the better as well? I can imagine penetration must feel way better now than before you had it done, unless the nerves are affected in any way. I used to have about a 6.5 to 7 inch (17/18cm) erection in the past, but not as much girth as you. I used to have about 12cm around girth (I guess that's about 5 inch around maybe a little less). Over the years the size has changed to about 15cm in length and the girth has only been affected around the glans but havent measured it lately. If I could remain at 15cm length and regain a little girth I'd be pretty happy. At least I'd have reliable erections again. The only thing I worry about now is that I still seem to be in acute phase once in a while where I have pain and worsening of the deformity. I hope this won't go on after surgery makeing the implant damage the inside even more if that makes sense. What's your experience with this? Do you still have pain or discomfort?

Popopo- I will do my best to answer your questions.... hopefully the answers help a bit.  The question of sensitivity is still a work in progress. I have only had intercourse once (usually recommended to wait until at least the 3 week post surgery date) and my wife said it was very enjoyable for her.  It was enjoyable for me as well, but it does feel different.  This is driven by the sheer stiffness of the penis when inflated... it is very hard.  Since I was just past the three week mark the implant aches a bit when fully inflated.  I believe the ache is actually the plaque being stretched while inflated.  Dr Eid said this would go away with time and accordingly my curve would lessen since the plaque was being stretched.  In my case he said he wanted me to inflate as many times as I could to get the plaque stretched as soon as possible. So bottom line was that sex was enjoyable but there is a bit of an ache.  Most others have said any ache/pain greatly diminishes with time and usually by the 3 month mark there is no pain whatsoever. I still have glans engorgement which allows me to keep all normal sensitivity in the tip of the penis which is great.

Penetration is much better post implant. My wife can clearly feel a difference and we were able to try different positions whereas before surgery we pretty much just stuck to typical missionary because I wasn't able to thrust deep enough for any other position due to the curvature and shortening.  This was one of the key drivers of me going the implant route since I did not have full blown ED yet.

In terms of Peyronies Disease recurring post implant I don't think that will happen. When I asked Dr Eid about that he said there was no way Peyronies Disease would come back because when inflated the tissue is stretched and the penis is elongated.  I guess, theoretically Peyronies Disease could come back and if one did not cycle appropriately the penis could perhaps shorten because the tissue wasn't being stretched enough but it would be best to ask a surgeon this question.  I don't know of the implications of acute vs chronic stage on implant... good question for a surgeon.

I think the best first step for you would be to have a phone/video consult with a couple of surgeons to ask questions and get their expert opinion.  Dr Eid does phone/video consults but there is a charge for these.... I want to say $250.00 but I am not positive on that.  I think other surgeons may do these free of charge but am not sure. Another surgeon highly respected and referenced on this board is Dr Andrew Kramer. There is a link to a listing of surgeons on the home page of this site that may help.

Thanks for your answers. What would be the cost of the implant itself? And do you think my insurance would cover it when I have it done in the US? I'm from Holland myself. I also must keep in mind that I probably need to have it replaced in my lifetime because I'm only 24. I hope the costs won't be too much for me.

I imagine each surgeon is different in terms of what insurance they will take.  For instance Dr Eid only takes Medicare and even though he was in network for my insurance I had to pay out of pocket and am fighting to get reimbursed on the back end.  I am sure other surgeons take insurance but you will have to contact their offices to determine what and how much is covered.  The costs will also vary by surgeon but if you are paying out of pocket you could pay up to $25,000.00 (excluding hotel and airfare).  For Dr Eid his services were $14,000.00; the coloplast Titan cost approx $8,500.00 and the surgery center and anesthesia were an additional $2,500.00.  

The life expectancy of the implant itself is estimated at 10-15 years but some have lasted longer and it will depend on how much you use it.
